Those working in healthcare policy and payment reform are used to navigating political changes and challenges. However, many didn’t anticipate the strength of recent attacks on the Direct Contracting Model that brought the program under intense scrutiny and threat. The shift to value-based care has long been a bipartisan effort, with both sides of the aisle agreeing on the need to evolve our health care payment and delivery system from one based on uncoordinated fee-for-service to one based on a whole person, total cost of care approach.
Care coordination is becoming increasingly important in U.S. healthcare for a variety of reasons, including the increased use of value-based care models, the behavioral care shortage and a boom in home healthcare.
The pandemic underscored the need for providers to lean into value-based care – physicians are paid according to the quality of care they deliver, as indicated by patient outcomes. As these models become more prevalent, it’s in providers’ best interests to ensure a successful patient transition and recovery, which is where care coordination comes in. On another note, there has been a continued shift in referral patterns to home health. Patients are increasingly opting to recover in the home instead of a hospital or rehab facility, which requires care coordination.
In today’s post-acute care landscape, growth and success will look different for everyone. Market conditions, resources and budgets, company goals – each of these plays a role in determining the go-to-market strategy for your organization. And to truly understand and define these areas, you need to first start by looking at the market intelligence and performance insights of your organization, your competitors, and your referral partners. Data is – or at least, should be – the central cog in your post-acute growth engine. And that data should power and accelerate the critical components of that engine – allowing your organization to reach your post-acute growth goals strategically and efficiently. Before we get into how data powers this engine, let’s first look at what the steps make up the engine itself.

The types of social determinants of health that medical providers may identify in their patients vary, meaning it will take numerous social determinants of health interventions to support patient wellness. But with these hospital- or clinic-based social service referrals being somewhat in their nascency, not every organization has a solution for every social determinant of health.
