The United States has the highest maternal mortality rate of any industrialized country, and it has increased in recent years. Other more common negative maternal health outcomes such as severe maternal morbidity are also on the rise. Improving the quality of perinatal care to address these poor maternal health outcomes is a pressing policy concern. More than 40 percent of births in the US each year are covered by Medicaid (exhibit 1), including 64 percent of births to Black birthing people, making changes to federal Medicaid policy or state Medicaid programs potential policy levers for addressing maternal health and maternal health equity. | | |
Attaining health equity remains a paramount objective for health care professionals aiming to transcend the impact of socioeconomic factors on individuals’ health outcomes. The Centers for Medicare & Medicaid Services (CMS) has emphasized advancing health equity to address care disparities. Despite this effort, many communities grapple with health care access and outcomes disparities, often exacerbated by administrative hurdles like prior authorization processes. Prior authorization can intensify the issue of health inequity further as it’s a significant factor in using social determinants of health (SDOH) to make vital care decisions. Studies have shown that prior authorization disproportionately affects low-income patients, people of color, and those living in underserved areas. | | |
Established healthcare organizations, whether it be health systems, accountable care organizations, or independent provider groups often grapple with the dilemma of fully embracing risk. Fears of taking downside risk and capitation payments are intimidating given the known unknowns regarding participating in fully risk-based programs, regardless of whether one operates within the Medicare Shared Savings Program or ACO Reach. Accommodating varying provider maturity levels and comfort with risk is crucial. By offering blended options, individual risk pools, and net neutral capitation payments providers can embark on their journey toward value-based care at a pace aligned with their tolerance and readiness. | | |
CareFirst BlueCross BlueShield and Children’s National Hospital in Washington, D.C., have entered into a value-based care alliance to address three key priority health conditions: behavioral and mental health, diabetes, and infant mortality. The first initiative involves addressing the youth mental health crisis. As part of this alliance, nonprofit CareFirst has increased the reimbursement for preventive and mental healthcare for primary care providers who go through specialized mental health training focused on depression, suicide prevention, anxiety, ADHD and eating disorders. | | |
On November 2, 2023, the Centers for Medicare & Medicaid Services (CMS) unveiled its 2024 Physician Fee Schedule final rule. It includes changes to greatly increase access to behavioral healthcare for Medicare beneficiaries. Because many compliance rules in behavioral health will be changing in 2024, and simultaneously, a new set of behavioral health professionals will be entering the Medicare program for the first time, there’s never been a better time for a compliance review on behavioral health. | | |
