Most primary care physicians (PCPs) are engaging with value-based care, with 69 percent participating in some type of alternative payment model. But a new report from Chartis Group has identified the PCPs leading this transition away from fee-for-service. Value-based care “Leaders,” according to the report, are PCPs who report participating in at least a partial-risk capitation agreement. Only about one-fifth (21 percent) of the over 300 PCPs surveyed for the report say they are in partial- and/or full-risk capitation agreements. The other 28 percent of value-based PCPs are considered “Experimenters.” Experimenters acknowledge having a value-based agreement, with accountable care organization (ACO) and patient-centered medical home (PCMH) being the most common agreements among all PCPs.
While the immediate fears, uncertainty, and isolation of the COVID-19 pandemic have practically disappeared, the mental health crisis sparked by these issues have not been so easily erased. Moreover, the number of people who need mental and behavioral health services continues to surge. October is National Depression Education and Awareness Month. The goal of this designation is to encourage universal health literacy regarding the signs and symptoms of depression and bring awareness to varieties of help and treatments available. But most importantly, the designation was created to highlight the need for more accessible and affordable mental health screenings. 
Over the past few years, we’ve witnessed dramatic acceleration in the movement of more healthcare into the home. With healthcare leaders estimating 20-30% of all Medicare spending could shift to the home in the next few years, that momentum is continuing in spite of sluggish permanent payer incentives. Moving care into the home provides value in terms of lower costs and better clinical and safety outcomes, but one topic that doesn’t get enough attention is the opportunity care at home provides to address health equity in our communities.
