Quality Corner: VCU Health Implements Red Rules
VCU Health is committed to becoming America’s safest health system, with zero events of preventable harm. Achieving this goal requires a culture of safety where ownership over patient and team member safety is the priority of every team member. While Red Rules are new to VCU Health, this is a well-regarded best practice across many highly reliable industries. The Red Rules are listed below and you can learn more here.

Handoff of Non-Infusing Blood Products
The following steps need to be performed with Every Product, Every Time
  1. The team member delivering blood products reads from the bag tag or cooler insert and says, “I have a blood product or blood cooler for [says] Patient Name and medical record number.”
  2. The team member receiving the blood product reads back from the product bag tag or cooler insert and responds “Correct, the blood product(s) is(are) for [says] Patient Name and medical record number.”
  3. The team member delivering the blood responds with either: “Correct” or “Incorrect”. If incorrect, contact the Blood Bank.
Blood Product Bag Tag Completion
Bag tag documentation is completed prior to administering any blood product that is not bar code medication administration scanned into the electronic health record (massive transfusion protocols, emergency release blood, and blood products administered during downtime).
Patient Identification
Prior to starting the blood administration process, validation of the patient's identity is performed by matching a minimum of two patient-specific identifiers (either patient's first and last name and date of birth, OR patient's first and last name and medical record number) on the identification band to that on the blood product bag tag.
