There is an opportunity for Seniors 65 and older to be vaccinated at the Mundelein Park District Community Center on Wednesday, March 24. This is in partnership with Jewel/Osco. Registration fills quickly. There is a limited amount of vaccine available, so please register immediately. When you click to sign up, If it says it is full, click again because sometimes when too many people are signing up at once it says it is full.

Please see the link below to register for the clinic. Please register immediately before the spots fill up. Once registered the vendor will contact you via email with the time of your appointment. Your confirmation for your second dose will be April 14 at the exact time slot as your first dose.

Only sign up if you are able to take both doses -first dose March 24 and second dose April 14. Click here to register

Copy of drivers license or state ID

Photocopy of health insurance card, Medicare part B card or Advantage Plan

PLEASE ARRIVE IN THE BUILDING 5 MINUTES PRIOR TO YOUR APPOINTMENT.

IF YOU COME EARLIER PLEASE REMAIN IN YOUR CAR.

Please wear a short sleeved shirt, and bring your walker, wheel chair or cane if needed because there will be some standing.

Only one care giver allowed in building per couple.

Are You Homebound and Need the Covid Vaccine?

Please call (847) 377-8130 and let them know you are homebound. There will be a program to address those homebound and they are asking you to call to get on their list of residents without accessibility.

Have You Filed For Your Annual Exemptions in Fremont Township?

There are 3 exemptions that need to be renewed on an annual basis:

· Senior Citizen’s Assessment Freeze

· Homestead Exemption for Persons with Disabilities

· Homestead Exemption for Veterans with Disabilities

If you are in need of assistance completing and submitting the exemption renewals, please call us for help. You will need to provide us the necessary documentation.

Required Documents

Each exemption requires the property be their principal residence.

To Qualify for the Senior Citizen’s Assessment Freeze:

Be 65 years of age or older.

Have a total household income of $65,000 or less in 2020.

Provide your 2020 tax return, 1040 or 1040SR for 2020

To Qualify for the Homestead Exemption for Persons with Disabilities

Provide a copy of your Illinois Disabled Person Identification Card or

Provide Proof of Social Security Administration Social Security Benefits or

A completed PTAX-343-A Form, Physician’s Statement

To Qualify for the Homestead Exemption for Veterans with Disabilities

Provide documentation from the U.S. Department of Veterans’ Affairs for the current assessment year, 2021, stating the service-connected disability.

Our door is locked so that we can maintain social distancing, but we are open. Our office is small and we are only allowing one resident in the building at a time. We have not closed during Covid. We continue to provide the vital services our residents need.
