Please complete and submit this ACH authorization form to CPP at cpp@nationwidechildrens.org or fax it to 614-722-2149. If you already submitted this form to CPP, you do not need to complete it again.

NEW Pneumococcal Conjugate Vaccine
The CDC has published the ACIP recommendation of Merck’s Vaxneuvance (PCV15) for pediatric pneumococcal vaccination. It should signal insurance companies to begin paying for Vaxneuvance when administered to patients at 2, 4, 6, and 12-15 months. As practices review the clinical information for Vaxneuvance we recommend reaching out to your major payers to make sure adequate payment is in place. The CPT code for Vaxneuvance (PCV15) is 90671.
