Dear Community,
Agudath Israel of Illinois, Hatzalah Chicago and Refuah 311 have joined together to form the Community Vaccine Alliance which will be working to get the COVID-19 vaccine to all eligible individuals and community members. We have created a joint survey which is intended to assess community need and plan inoculation sites as vaccines become available.
The Community Vaccine Alliance is following the guidelines of the Illinois Department of Public Health and Chicago Department of Public Health and will be providing vaccinations in a multi-phased approach. Please complete the below survey and you will receive a notice with an application when you are eligible to make a vaccination appointment and there is sufficient supply. The Community Vaccine Alliance will move to new phases as sufficient doses of the vaccine become available.
