ATTN: Training Course Sponsors in Washington State
Washington Certification Services (WCS) is proud to announce that updated fillable forms are now available on our website. While the majority of the required information remains unchanged, we trust that trainers will find the updated forms to be clear, functional, and more user-friendly.
What forms are new and available?
- Training Roster
- Training Roster Attachment
- Request for Course CEU Evaluation
- Request for Conference CEU Evaluation
- Request for Self-Paced Training CEU Evaluation
What's new?
Relevancy Documentation:
Formerly a small section on the Request for CEU Evaluation forms, two declarations related to content relevancy are now required to be submitted as attachments.
Certificates of Completion:
As indicated on the new forms, a copy of the certificate of completion is now required to be included in the submission for Course and Conference CEU evaluations.
Self-Paced Training CEU Requests:
This form has been updated to match the modern delivery methods of self-paced training. We encourage trainers to submit their self-paced training courses for CEU approval in Washington state.
