Wisconsin MCO Care Plans Now Available in WISHIN
The Wisconsin Division of Medicaid Services (DMS) has been working with WISHIN and managed care organizations (MCOs) to develop a standardized format to share the MCOs’ care plans for their SSI members through WISHIN. This creates a single point of access for providers and payers, including DMS. After initial workgroup sessions, WISHIN and DMS worked to establish a uniform and consistent care plan format that all MCOs would use to send care plans to WISHIN.
The goals of this project are to:
· Enhance the sharing of health information across the healthcare continuum to improve quality of care and cost management
· Provide access to care management information to fill data gaps related to medication, member goals, and social determinants of health
· Support providers’ efforts to meet Meaningful Use requirements, specifically measures surrounding the reconciliation of clinical information
· Expand access to discrete clinical data for quality-measurement purposes
Project status:
· Molina, GHC-EC, and United HealthCare are live with sending their care plans to WISHIN Pulse.
· My Choice Wisconsin is scheduled to go live by June 7.
· MHS, Network Health, and iCare are scheduled to go live the week of June 14.
· Anthem is scheduled to go live the week of June 28.
· WISHIN and DHS plan to close out of the project in July.
