Under the updated policy beginning April 14, 2025, individuals applying for Social Security Disability Insurance (SSDI), Medicare, or Supplemental Security Income (SSI) who cannot use a personal my Social Security account can complete their claim entirely over the telephone without the need to come into an office.

New York Connects started back in 2004 as a pilot program and went statewide in 2006, thanks to the New York State Office for the Aging and the Department of Health.

Then, in 2017, it partnered with Independent Living Centers (ILCs) across New York State as an effort to to enhance access to long-term services and supports (LTSS) for individuals of all ages and abilities, including those with disabilities.

Need help with home care, transportation, housing, or benefits? New York Connects has your back.
