Cardiovascular Disease Prevention Regional Coordinator

The Wyoming Department of Health, Chronic Disease Prevention Program, is accepting applications for two (2) regional contractors throughout Wyoming to coordinate chronic disease prevention and management at the regional level. The purpose of this competitive application process is to select two (2) regional sites throughout Wyoming to support the CDPP in implementing evidence-based strategies to prevent and manage cardiovascular disease (CVD), specifically hypertension and high cholesterol, in high-burden communities. The anticipated contract term will be from January 1, 2021 through June 30, 2022.
