To our case managers: We appreciate all you do for our community and the vulnerable population you serve. In order to continue to assist and strengthen our efforts, we would like you to participate in a short survey. This five question survey should not take more than a couple of minutes to complete but will make a great impact in guiding the efforts of the Continuum of Care. Thank you for participating!
Because of our community partnerships, we now have an online Coordinated Entry form sending responses directly to 2-1-1.

We have all seen what outstanding work we can accomplish together, especially through this pandemic. Now more than ever, our processes should consider the safety and well-being of our neighbors and our staff members. Implementing an avenue for Coordinated Entry that allows remote access benefits everyone. It is because of cooperative teamwork that we continue to improve our systems. We thank you all and encourage you to please continue to use the Online Intake Form.
