Open Enrollment is here! Go into Paycom to adjust your benefit details between Wednesday, 11/1/23 through Friday, 11/17/2023; changes and updates will take effect on 1/1/2024.
Annual Open Enrollment is your opportunity to review, update, or select your benefits for the upcoming year, ensuring they match your needs and preferences. This is the time to choose the best coverage for you and your family.

Don't Forget to Get Your Flu Shot!
|

We are at a 50% completion rate as of November 1st -
Don't forget to submit your proof of Flu Vaccination to HR!
To help make this process as effective and efficient as possible, several methods are being offered to receive and submit documentation of your vaccination. Please note, it is each individual’s responsibility to get vaccinated and ensure vaccination records are submitted to the HR department prior to the deadline of 11/17/2023 at 5:00pm. If you would like to authorize the FCH Nursing team to submit your vaccination record to the HR department on your behalf, you must complete the attached ROI and bring it with you to your vaccination appointment.
Methods of receiving and submitting proof of vaccination include:
Process for Receiving Vaccine at Any FCH Clinic: To receive the vaccine, employees can simply check-in with Reception and start the following process:
1. Patient (Full Circle Health Employee) presents to Reception with ID and insurance card to request a flu shot.
2. Normal check-in process occurs (verify information / insurance etc.)
3. Nursing calls patient back to the room and administers vaccine(s) per standard protocol.
4. Appointment is finished and Patient (Full Circle Health Employee) leaves to return to work.
5. If the Patient (Full Circle Health Employee) has not completed an ROI, the Patient (Full Circle Health Employee) submits a copy of their vaccine record to HR@fullcircleidaho.org or may retrieve vaccine records as described in the next section.
Process for Retrieving Immunization Records at FCH:
2. Click “Your Menu” at the top left-hand side of the screen.
3. Click “Health Summary” on the left-hand side of the screen.
4. Click on the “Immunizations” tab.
5. Locate the vaccine information you wish to share and click “view details”.
6. Click the printer icon on the top right-hand portion of the screen.
Process for Receiving Vaccine at Non-FCH Clinics: For those who receive vaccine at a non-FCH clinic, please scan a copy of your immunization card/record to HR@fullcircleidaho.org.
The vaccine must be obtained and record submitted to HR before the close of business on 11/17/2023. The HR department and / or supervisors will provide notification to non-compliant staff of the need to comply or be subject to disciplinary action, up to and including employment separation or termination of contracted services. For individuals who are unable to receive the flu vaccine due to medical or religious reasons, those exemption requests were due to HR by 10/13/2023. If your hire date was after this timeframe, please contact HR as soon as possible.

Clinical Applications Specialist (CAS)

The successful Clinical Applications Specialist (CAS) acts as a “champion of change” and facilitates Full Circle Health's efforts to continuously improve the effectiveness of the electronic health record (EHR). The CAS strives to transform the delivery of healthcare through technological improvements and optimal process design.
|
|
The CAS enables Full Circle Health's effective use of the EHR and other applications through administration, support, training and process improvement.
Schedule: 8:00am-5:00pm, Monday-Friday.
Experience
- 2-3 years working in a clinical setting providing EHR system support.
|
|
This is a quarterly award given to an employee who is nominated for their outstanding work that impacts our organization and community. 'Patient Voice' pilot project in collaboration with Dartmouth Institute and Pacific Source:
Project Mission
To improve the health and healthcare value of FCH patients and professionals, who are living with complex chronic conditions – and who may be challenged by social determinants of health – by matching health and community service needs with resources to meet those needs effectively & efficiently.
Project Vision
In 5 years, to be working with all FQHCs (Federally Qualified Health Centers) in Idaho to master the person’s/ patient’s/ population’s mesosystem by co-designing and implementing a learning health system that coproduce’s optimal health status, high value healthcare and that continuously improves key measures of health, healthcare value and professional staff’s wellbeing and joy in work.
To improve results for the targeted high-risk population, the following innovations are in progress:
- Use patient voice (via a pre-visit questionnaire) – goals & concerns, trends in health & wellbeing -- to guide assessment, shared decision making, service planning, navigation & outcomes tracking
- Start a collaborative learning “community” network (health, social, and lay people) to learn and improve key results over time
- Start a person-centered registry to track key data
Strategy:
We are using a Co-design model with the Meridian Clinic team of physicians, nurses, care manager, behavioral health, community health worker, clinical pharmacist, and population health leaders, partnering with Dartmouth and Pacific Source key person(s) to determine workflow and implement the 3 innovations above. We aim to “Start small and simple” by capturing the patient’s voice to partner with the patient/family to plan, coordinate & deliver needed services across the mesosystem. We have developed key performance measures to track the impact on key outcomes, healthcare utilization, healthcare expenditures, and patient experience..

Slow-Cooker Hawaiian Pork Tacos
|
|
November is National Diabetes Month. Three practices that can help manage our blood sugar include: lowering overall carbohydrate intake in our diet, increasing fiber intake, and exercising.
General recommendations for fiber intake are: 25 grams per day for women and 35 grams per day for men. Most Americans get about 50% of the recommended fiber intake due to greater consumption of highly processed and fast or convenience food, and lower intake of whole grains, and fruits and vegetables. Include higher fiber foods each day like fruits and vegetables, beans and lentils, nuts and seeds, higher fiber cereals, whole grain breads, pastas, and grains like bulgur or quinoa, oatmeal, and steel cut oats. A healthy practice is making 50% of your plate fruits and vegetables.
General guidelines for carbohydrate consumption are: aiming for 30-40 grams or less of carbohydrate per meal and about 15-gram carbohydrate snacks if needed. Limiting consumption of sugar including sweets, candy, soda, and juice.
Exercise also helps our blood sugar. General recommendations for exercise include: doing at least 150 minutes of moderate to vigorous activity per week. For example, taking a brisk walk most days of the week for 30 minutes for better blood sugar control and heart-health. Taking a short walk following meals can also help blood sugar control and can be a peaceful and enjoyable experience for our mental health as well.

Ingredients
· 3 ¾ lbs. lean, boneless pork shoulder
· ½ tsp. black pepper
· ¼ tsp. ground ginger
· 1 tsp. cumin
· 1 medium onion (sliced)
· 2 cloves garlic (minced)
· 1 cup pineapple juice
· 1 cup white wine
· 11 small high-fiber tortillas (warmed)
· 1 cup shredded lettuce
· Avocado (optional)
Directions
· Place pork shoulder in a slow cooker. Sprinkle meat with pepper, ginger, and cumin. Add onion and garlic on top of roast. Pour pineapple juice and wine over roast.
· Cover and cook on high for 4-6 hours (or on low for 7-8 hours). Drain liquid and shred pork with fork.
· Scoop about 1/2 cup pork into each tortilla. Top each taco with shredded lettuce and any additional toppings of your choice.
