Columbus Medical Association affiliate, the Healthcare Collaborative of Greater Columbus (HCGC), is working collaboratively with other partners across Ohio, to engage and recruit primary care practices to participate in a statewide initiative, Heart Healthy Ohio.
This initiative builds on the CARDI-OH initiative focused on increasing primary care practices utilization of best practices to manage cardiovascular health, patients’ blood pressure, and patients with hypertension. Criteria for practices can be found here.
Heart Healthy Ohio is funded by the Agency for Healthcare Research and Quality (AHRQ) and is aimed at improving the cardiovascular health of patients across Ohio, with specific emphasis on minority populations.
