Crypt Opening For Unclaimed Remains - Aug. 5 | |
The next scheduled opening of the crypt at Memorial Park Cemetery is Friday, Aug. 5.
If you have unclaimed remains to be placed in the crypt, please complete the following steps:
- Verify that all required outreach to the family has been completed and documented according to state law and company protocol.
- Transfer cremated remains from the original urn into a black, plastic temporary urn.
- Make sure plastic temporary urn is properly labeled using cremated remains labels.
- Add transaction line to the deceased’s Chain of Custody form that shows the cremated remains being “transferred to other FH/NFSGI Facility”, the releasing party being the FH, and receiving party being Memorial Park Cemetery, Loc. #51 via USPS. Keep the original on site and mail a copy with the cremated remains. Memorial Park staff will add a transaction line upon receiving to show custody transfer and another transaction line when placed into the crypt and send back to you once complete.
- Ship according to USPS standards and company protocol directly to Memorial Park Cemetery, Attn: Julie Gould, 3616 SW Sixth Avenue, Topeka, KS 66606.
Additionally, the Request of Unclaimed Cremated Remains from Cemetery Storage form can be found in the Paycom Document Library and should be submitted for any families wishing to claim remains from the crypt. That request needs to be submitted no later than Wednesday, Aug. 3.

Entering Adjustments & Discounts in SRS |

When entering an Adjustment in SRS, please start your note with “Increase [item]” followed by the remainder of your note (Ex: “Increase Newspaper cost to $150.00, invoice greater than contract”).

When entering a Discount in SRS, please start your note with “Decrease [Item]” followed by the remainder of your note (Ex: “Decrease Urn cost, family changed mind).

If the Discount is for a write-off (i.e. FDIC decides the family will not be charged for the item), please start your note with “Write-off [item]” followed by the remainder of your note (Ex: “Write-off Flowers, Our Promise, flowers not delivered to service).

Instead, navigate the platform with confidence and ease. In addition to this enhanced user experience, you can now schedule care within the platform, based on your preferences. Once logged in to the web or mobile platform, look for the calendar icon labeled ‘Schedule Care’ within the access bar.
