This month, HIIN The Know focuses on patient and family engagement (PFE), a critical part of the Partnership for Patients (PfP) program. Hospitals are tasked to implement five PFE best practices that lead to improved outcomes. These practices are listed in the table below. We have also included all HSAG HIIN hospitals' progress toward meeting these metrics relative to all PfP hospitals.

The Data Advantage Corner
HIIN Comparisons Dashboard
The HIIN Comparisons Dashboard is now available on the HSAG HIIN Secure Data Portal! The interactive dashboard allows your hospital to select from a list of specific measures to display a comparison of your hospital’s performance and progress toward improvement goals in contrast to three peer groups: an aggregate of all the hospitals in the HSAG HIIN, a Hospital Characteristics Peer Group, and a Regional Peer Group.

To find the HIIN Comparisons tab:
  • Log on to the HSAG HIIN Secure Data Portal.
  • Select the Dashboards tab at the top of the Secure Data Portal homepage.
  • Locate the HIIN Comparisons tab next to the HIIN Measures tab (please note, the dashboard will take approximately 60 seconds to load).
  • Select your hospital from the Hospital filter, then select a measure from the Measure Names filter.
For detailed information on calculations and how to interpret the comparisons, please select the green "FAQ" button on the dashboard. If you have any questions or need further clarification regarding the dashboard, please contact your HSAG HIIN CIA or [email protected].
Hospital Readmission Reports Now Available
The Readmission Feedback Report provides a summary of your hospital’s Medicare Fee-for-Service (FFS) readmission data through Q3 2017 and provides overall and stratified readmission rates, allowing your hospital to identify key areas of focus and improvement. Please note, some data may have changed slightly compared to previous Hospital Readmission Feedback Reports. These changes reflect recent measure specification updates from CMS associated with the classification of planned readmissions. To access your Readmission Feedback Report, select the Reports tab on the HSAG HIIN Secure Data Portal .
Reminder—PFE Assessment
To ensure that education and support is meaningful to your hospital’s patient harm reduction efforts, HSAG HIIN would like to receive an update on your hospital’s current PFE practices. Please take a moment to complete a short PFE assessment by logging into the HSAG HIIN Secure Data Portal. If you are uncertain whether your hospital has implemented a particular PFE practice, please consult with the person most knowledgeable with your PFE efforts.To access your hospital’s PFE assessment, select the PFE Assessment tab on the HSAG HIIN Secure Data Portal .
