Welcome to the September Issue of HIIN The Know !
September 2018 marks the two year anniversary of the Health Services Advisory Group Hospital Improvement Innovation Network (HSAG HIIN), a partnership of HSAG and Hospital Quality Institute (HQI). In this issue, HSAG HIIN celebrates the incredible work and successful efforts in harm reduction by our HSAG HIIN hospitals. In recognition of the value of our collective harm reduction efforts, the Centers for Medicare & Medicaid Services (CMS) is extending our HIIN work through March 2019.

Key clinical topic areas that have reached or are approaching the target of 20 percent improvement include Healthcare Associated Infections (HAIs) and Falls with Injury. Nevertheless, there are topics that still require our dedicated efforts—Ventilator-Associated Event (VAE), Venous Thromboembolism (VTE), Sepsis, and the three adverse drug event (ADE) measures are below the 20 percent improvement target. In addition, 30-day Readmissions are below the 12 percent improvement target. The performance of each HIIN clinical topic is presented in the Analytics section.

September is also a month for recognizing Sepsis. See links to the National Content Developer (NCD) partner Sepsis Alliance for training resources and materials you can use for your Sepsis recognition days.

As part of the HSAG HIIN, your hospital has access to sepsis and fall prevention resources to support your efforts. Please reach out to your Clinical Improvement Advisor (CIA) to receive your personalized, no-cost assistance today!

The Data Advantage Corner
HAI Reports Now Available
To continue monitoring progress on HAIs, HSAG HIIN will disseminate another HAI Feedback Report featuring data through Q1 2018. The report provides an overall summary of HAI measures, allowing your hospital to identify critical areas of focus and improvement to support progress toward the bold aims of the Partnership for Patients HIIN program. To find your hospital’s HAI Feedback Report, select the Reports tab on the HSAG HIIN Secure Data Portal .

Reminder—Access the HIIN Comparisons Dashboard
Don’t forget to review the HIIN Comparisons Dashboard on the HSAG HIIN Secure Data Portal. The interactive dashboard allows your hospital to select from a list of specific measures to display a comparison of your hospital’s performance and progress toward improvement goals in contrast to three peer groups: an aggregate of all the hospitals in the HSAG HIIN, a Hospital Characteristics Peer Group, and a Regional Peer Group. To access your hospital’s comparison dashboard, select the Dashboard tab on the HSAG HIIN Secure Data Portal .
