Join the Home-Based Primary Care Special Interest Group at AGS

If you are attending the 2022 American Geriatrics Society (AGS) Annual Scientific Meeting in Orlando from May 12 to 14, join HCCI’s Home-Based Primary Care (HBPC) Special Interest Group (SIG) for a May 13 strategy meeting. All interested in home-based medical care are invited to attend this session at AGS to help define what this group’s goals will be as it moves forward.

Led by Co-Chairs Melissa Dattalo, MD, Assistant Professor, Division of Geriatrics and Gerontology, University of Wisconsin-Madison, and Namirah Jamshed, MD, Associate Professor, Medical Director House Call Program, UT Southwestern Medical Center, the Home-Based Primary Care SIG aims to ensure every medically complex patient who is homebound or home-limited has access to quality HBPC that results in a profound improvement in medical outcomes, patient and family experience, and healthcare spend.

HCCI has launched several new courses and workshops, in addition to the virtual Essential Elements of Home-Based Primary Care™ Workshop mentioned above, that cover important home-based primary care topics, including Diversity, Equity, and Inclusion (DEI), Patient Assessment, and more:

Diversity, Equity, and Inclusion for Home-Based Care is a free on-demand course, supported (in part) by the Josiah Macy Jr. Foundation, that addresses the complex topic of health equity by discussing power and privilege, implicit and explicit bias, inferred cultural preferences, and more.

Patient Assessment in Home-Based Primary Care is an online course that lets you shadow Dr. Paul Chiang , HCCI’s Senior Medical and Practice Advisor, who has made over 35,000 house calls, as he prepares for and conducts a simulated visit with a patient and his caregiver. The two-day Advanced Applications of HBPC™ Workshop teaches experienced home-based primary care providers and staff actionable strategies with a value-based care lens in staffing, infrastructure, and data; patient identification; cost-effective management of medications and conditions commonly found in patients; quality measurement and reducing gaps in care; and more.
