NCTH New Year News!

'23 Reflections and '24 Projections

Freehold Family Health Center

If you are interested in making an appointment for you or a family member, and want to learn more, contact our Bilingual Community Health Worker, Karen Ubeda, at 732-547-7274 or Karen.Ubeda@vnahg.org

Check out the flyer for more details!

NCTH continues our partnership with the Freehold Family Health Center (FFHC). We educate the community about all of the great services offered there, and connect people to appointments.

FFHC is located at 597 Park Avenue. Whether it’s podiatry, prenatal care, mental health referrals, COVID-19 testing and vaccines, or primary care for the entire family, you can find it at the FFHC.

The center is also federally qualified, so those who are uninsured will be given an affordable rate for their care.

Freehold Intergenerational Community Kitchen

We had so much fun in 2023 continuing our Community Kitchen program! Since inception, the program has engaged 29 participants. Together, kitchen members learn new information about healthy eating and food safety, review recipes, and cook meals to take home to their families. We are thankful to our generous funders, Board of Trustees, St. Peter's Church (for hosting us), FICC members, and all of our wonderful kitchen members.

NCTH seeks funds to continue this program in 2024. Stay tuned, and please reach out to our team if you would like to be involved in this project.

This year's peppers and herbs were grown in the Freehold Borough Community Garden!

Thanks to the Freehold Area Health Department, and Dr. Abdel of the Freehold Family Health Center, for educating our participants on Culinary Medicine, Safe Food Storage and Food Allergies!

Disability-Centered Patient Advisory Council

This initiative is funded by an Inclusive Healthy Communities Grant from the Division of Disability Services, New Jersey Department of Human Services.

NCTH is working to expand healthcare quality and access for individuals with disabilities at the Freehold Family Health Center.

With two meetings under our belt, and a third just days away, the 'Patient Advisory Council' is launching! We are thrilled to have such a wonderful group of people sharing their passions for quality healthcare and disability inclusion. We are fortunate for the opportunity to collaborate with the Freehold Family Health Center on this project.

Currently, the Council is working to create a shared vision of disability inclusion in healthcare, rename the group to reflect members' voices, and propose an action plan for positive change at the Freehold Family Health Center.

If you are a person with a disability, a caregiver, or a professional in the field of disabilities, this opportunity could be for you! Members of the council meet regularly to build an action plan for inclusion at the Freehold Family Health Center.

If you have any questions or would like to join this project, contact Carly Trill @ Carly.Trill@vnahg.org / 908-692-7338

Bikability Walkability

In 2023, the Bike/Walk committee held bike rides and walks monthly through the Spring, Summer, and Fall! In total, the community convened for 12 intergenerational bike rides/walks, traveling 49.5 miles together, thanks to this group! A special thanks goes to our awesome volunteers who led this effort - Wagner Menendez and Megan Valeski!

We look forward to the year ahead for more opportunities to bring the community together through physical activity. Do you have a passion for biking and/or walking? Are you looking for ways to build community? The Bike/Walk committee would love to have you! Reach out to Carly @ Carly.Trill@vnahg.org OR 908-692-7338 for more information.

Freehold Intergenerational Community Council (FICC!)

As you may know, FICC is a wonderful group of individuals who represent multiple generations and cultures. Projects such as the Community Kitchen are a product of FICC.

FICC is growing! In November '23, we welcomed 11 new members to the Council. This month, the new members will begin their Intergenerational Leadership training, with support from NCTH Staff, Board Members, and current FICC members. Please join us in welcoming our new members!:

Strategic Planning

NCTH's Board of Trustees and Staff Members are well on their way to completing a 5-year strategic plan. The team is able to draw from recent Community Needs Assessment and Focus Group data, as well as City Health Dashboard Data, to update our original Blueprint for Action. Thank you to Janus Solutions, Freehold Borough residents and community members, and all of our partners for supporting us in this process! We look forward to advancing our work with you and sharing our finished product.

Tackling COVID and Flu Season!

In partnership with Casa Freehold and the Freehold Area Health Department, we were able to provide 42 COVID-19 vaccines, 87 Flu vaccines, as well as shingles and Tdap, in 2023 through our monthly clinics! These vaccines were offered for FREE and supported those with/without health insurance. An evening clinic schedule and bilingual staff and volunteers expanded access even further to our community. Further, we partnered with MOCEANS Centers for Independent Living to expand access for individuals with disabilities at our clinics. We look forward to providing more vaccine access in 2024!

The NCTH team thanks you for being part of our coalition!
