CrossRoads: Dynamet Lesser TMT Fusion
CrossRoads continues their targeted assault on Treace Medical by launching their Dynamet Lesser TMT Fusion System. DynaMet provides a comprehensive suite of instrumentation and implants that can be used in conjunction with the dynaBunion™ to treat any 1st-3rd TMT pathologies. It will be interesting to see how Depuy Synthes manages the CrossRoads purchase moving forward. This product was clearly in the Crossroads pipeline prior to the acquisition. We have seen this story before. Synthes bought BME, an up-and-coming Nitinol focused company with unique technology for the foot and ankle space. The consultants faded away and the innovation behind BME died. Plans for nitinol plates and screws never got off the ground.

CrossRoads sales force was made up of several independent foot and ankle specific distributors who in addition to CrossRoads, carry many other foot and ankle product lines.  Will those distributors be brought over to Depuy Synthes? Will some be absorbed and hired by the local trauma teams?   Depuy Synthes previously recruited and hired away big time Foot and Ankle Reps from Stryker and Wright Medical for large sums of guaranteed dollars only to scrap the program and let them all go only a few years later. Some hung on with local trauma teams. 

Part 2 of the Depuy Synthes foot and ankle project needs to be different if they want it to work. Trauma teams managing foot and ankle specialists has not worked in the past. Treace is vulnerable if this deal is handled properly. Treace is getting an average national ASP of $5100 for the Lapiplasty System. Depuy Synthes is on every contract in the country, offering their system with equal if not better technology with a price $2000 less than Lapiplasty could create a serious problem for Treace if Synthes focuses on cost savings.

This is a big story to follow into next year. Will DePuy Synthes become a foot and ankle force or will they fade back into their old trauma ways?
ACQUISITIONS
Is AR Coming to the STAR Total Ankle?

Augmented Reality (AR) is coming to Enovis/DJO in newly acquired Insight Medical. DJO announced the acquisition last year, but the deal was completed earlier this week. The Insight AR System is FDA cleared for Knee, Uni Knee, and Hip Arthroplasty. The system uses a headset for intraoperative guidance in planning cuts and desired implant placement. CEO, Nick van der Walt of Insight had this to say about the acquisition:


“We’re thrilled to partner with a leading MedTech company that has an impressive growth trajectory,” said Nick van der Walt, CEO of Insight. “DJO and Insight have a shared vision of improving clinical outcomes with technology in a streamlined, cost-effective way. We will build on the current capabilities of ARVIS in knee and hip arthroplasty and expand to other indications in orthopedics.”
DJO is one of the first Orthopedic Companies with a massive focus on foot and ankle to have access to AR technology. Total Ankle replacement would be an obvious place for ENOVIS to differentiate themselves, possibly giving the STAR implant a leadership role in some new technology. PSI is now required to play in the total ankle market. Stryker, P28, Exactech, and In2Bones, all have a PSI platform and by year end both ENOVIS/DJO and Smith and Nephew will have a patient specific platform of their own. Wright Medical was the first with Prophecy PSI and they took over the market. The Infinity, in design, is the same implant as most of the rest of the primary implants in the space. It will be interesting to see if the addition of PSI to theses other implants will begin to take market share from Stryker.
IF AR is the next big thing in Orthopedics, ENOVIS/DJO could lead the charge in AR Foot and Ankle. It makes sense and as Wright Medical has shown, being first matters. Intraoperative software for complex ankle procedures could change the game without the massive capital expense of total joint robots, like Stryker’s MAKO and Zimmer’s ROSA. Those million-dollar, capital intensive systems have the potential to become obsolete in the next few years if AR performs in joints as it has in Spine. In addition, it could lessen the amount of radiation used in these big whack foot and ankle cases by providing on demand information to correct deformity.

As for ENOVIS/DJO, the foot and ankle leadership team in Dallas is excited for the future and AR could be the play to take them to a different level.

RedPoint Medical 3D – The Better Bunion
RedPoint is taking two of the biggest trends in Foot and Ankle and combining them to create a company focused on 3D patient specific guides for rotational Lapidus correction. 

The Better Bunion System harnesses the powers of patient specific devices, Artificial Intelligence enabled software correction, and additive manufacturing to generate a cut guide that creates the perfect correction, every time. The system includes a simple, streamlined technique for multiplane correction. Once the deformity is corrected, the surgeon can use any hardware of their choice for Lapidus fixation.

RedPoint Medical was founded out of St Petersburg, FL by Dr. Adam Perler DPM and his partner, engineer James Spitlzer. RedPoint has put together a team of surgeon advisors (Adam Schiff, MD, Lee Hlad, DPM, Eric Nowak, DPM, and Eric Tan, MD) headed up by Duke’s Sam Adams, MD. The goal of this company is to make PSI for Lapidus and other complex foot and ankle procedures as instrumental as it is for total ankle arthroplasty. 

Questions FAN has about RedPoint

Is this technology proprietary? 
RedPoint is a software-based company that is built to sell, and hopefully sell quickly to an organization looking to compete in the rotational space with the newest technology. Are CT cut guides for foot and ankle procedures patent protected? What is stopping a company who has similar protocols in place for Ankle and Shoulder Arthroplasty cases from competing in this market?

Will hospitals and ASC’s pay for stand-alone Lapidus cut guides? 
PSI planning for the ankle arthroplasty on average fall in the range of $1000 to $2000 for the guides and pre-op surgical plan. The company creating the system gets the hardware pull through for the $12,000 cases. RedPoint is looking to receive a higher amount of dollars for the guides, instruments and patient plans than Ankle PSI because their will be no way to pull through implants.

Do surgeons want to deal with the time and expense of CT protocols for their Lapidus cases?
I think the answer to this question is very subjective and will elicit a ton of responses across the board. Some guys will bitch about implant costs and how two crossing screws can do the job just fine - this product is not for them of course. Others may honestly appreciate the help if this can make them more confident in doing a better job for their patients.

But what about Lapiplasty, Dynabunion, and Lapifuse users? Will this system convert them? That is the million-dollar question.
