Whole-Person Care Starts Here: Meet the GeorgiaUnify Resource Directory

Health isn’t just about medical care—it’s about the full picture.

With the launch of the GeorgiaUnify Resource Directory, GaHIN is making it easier for care teams and community partners across Georgia to work together and support whole-person health.

Whether someone needs housing, food, behavioral health support, or transportation, the directory connects providers to trusted resources—and tracks referrals to make sure no one falls through the cracks.

This is how we close care gaps. This is how we care better.

Closing the Loop, Opening New Doors: GaHIN and United Way Partnership

Georgia Health Information Network (GaHIN) and United Way are joining forces to reshape how care is delivered across the state. By bringing United Way of Greater Atlanta and United Way of Central Georgia’s 211 Database into GaHIN’s statewide health information exchange, this partnership creates a seamless bridge between healthcare providers, social care organizations, and public health agencies.

This collaboration, with our GaHIN technology partner, Ready Computing, sets a new standard for whole-person care, making it easier than ever to connect Georgians with vital resources. With a focus on building closed-loop referral networks at the community level, GaHIN and the United Way are ensuring no one is left behind. This initiative paves the way for a healthier, more connected Georgia.

The integration of the 211 Database into GeorgiaUnify will provide healthcare and social care organizations with real-time, actionable data to:

  • Identify social care resources across Georgia.
  • Seamlessly connect individuals to the services they need.
  • Ensure closed-loop referrals, where every referral is tracked, managed, and completed.
  • Enhance reporting and analytics for better decision-making.

Stronger Care, Smarter Decisions: GaHIN Powers Medicaid Provider Access

GaHIN is unlocking a new era of access for Medicaid providers—bringing critical patient data right to their fingertips, at no cost. Now available through the Georgia Medicaid Management Information System (GAMMIS), GaHIN’s secure health information exchange lets providers quickly view a patient’s medications, procedures, labs, immunizations, and more, all aggregated into a clean, easy-to-read record.

Better insights mean better, faster care—and fewer barriers for those who need it most.

By removing technology and cost barriers, GaHIN is leveling the playing field for providers across Georgia, especially those in rural and underserved areas. Medicaid clinicians can now close gaps in care, reduce preventable hospital visits, and make smarter decisions for patients—turning every point of care into an opportunity for better outcomes.
