All members (including dependents) who attend a medical visit to a HealthWise location from June 4th, 2021 to July 30th, 2021 are eligible to win and can submit one entry per visit.

Schedule an appointment to your local HealthWise Medical Clinic
Show up and complete your visit
Submit an entry to be entered to win!
Upon completing your medical visit, ask your medical assistant how you can enter to win. We have a designated QR code that will link you to the contest entry form from your mobile device, or the link can be sent to you in an email.
