Athletics
Wolverine Athletics Employs New PRIVIT Athletic Document System
for Middle & High School Athletes
There are three forms required by the state of Georgia for participation in middle and high school athletics. The three forms are a yearly GHSA Physical Form, Concussion Awareness Form, and Cardiac Arrest Awareness Form.

This year, PACS will begin using PRIVIT to electronically store and maintain these required athletic forms. Each family of a middle or high school athlete is asked to do the following:

  1. Please read and follow the directions in the PRIVIT welcome letter. (We have found that it might be easier for you to print out the directions to follow them.) You will be given a link to our PRIVIT site to register as a parent or guardian.
  2. You will then need to add each child as a “member”. You will be required to fill out the first page of the Pre-Participation Physical form regardless of whether you have already turned in a 2019-20 form to the office. Note - once you have completed this page of the form, it will be automatically stored and updated for future years at PACS.
  3. If you have not turned in a Physical form for 2019-20, you will be asked to take a picture of your completed form and upload it directly to PRIVIT. If you have already turned in the 2019-20 Physical form, PACS will upload the form once you have completed Steps 1 and 2.
  4. Please read and digitally sign the Concussion Awareness and Sudden Cardiac awareness forms.
