called me heavenward in Christ Jesus.

Wolverine Athletics Employs New PRIVIT Athletic Document System
for Middle & High School Athletes
There are three forms required by the state of Georgia for participation in middle and high school athletics. The three forms are a yearly GHSA Physical Form, Concussion Awareness Form, and Cardiac Arrest Awareness Form.

This year, PACS will begin using PRIVIT to electronically store and maintain these required athletic forms. Each family of a middle or high school athlete is asked to do the following:

  1. Please read and follow the directions in the PRIVIT welcome letter. You will be given a link to our PRIVIT site to register as a parent or guardian.
  2. You will then need to add each child as a “member”. You will be required to fill out the first page of the Pre-Participation Physical form regardless of whether you have already turned in a 2019-20 form to the office. Note - once you have completed this page of the form, it will be automatically stored and updated for future years at PACS.
  3. If you have not turned in a Physical form for 2019-20, you will be asked to take a picture of your completed form and upload it directly to PRIVIT. If you have already turned in the 2019-20 Physical form, PACS will upload the form once you have completed Steps 1 and 2.
  4. Please read and digitally sign the Concussion Awareness and Sudden Cardiac awareness forms.
*Additional Notes on PRIVIT:

  • Once you have set up an account with PRIVIT, future interactions with the system will be much easier and will only require you to update your information.

  • Once you have set up your account, please go back and check to be sure you do not have any outstanding documents.

  • If you have already turned in a paper copy of your child's 2019-2020 physical, the school cannot upload your copy until you set up an account. Please set up your account as soon as possible.

During his 24 years as a head varsity baseball coach, Frank Vashaw has led 20 of his 24 teams into the post-season playoffs: 13 teams to the Sweet 16, 10 in the Elite Eight, and 5 in the Final Four. Two of his teams have played for the state championship.
With a lifetime coaching record of 475-277, he has been named Coach of the Year sixteen times by various athletic and news organizations including Region 1-AAAAA, Region 7-AAAAA, The Georgia Dugout Club, The Gwinnett Dugout Club, and the Gwinnett Daily Post. He has also served as an associate scout for the Cincinnati Reds and has taken the Americas Team to play a series of games in Italy. Coach Vashaw takes over as varsity head coach from Phil Roberts who led the Wolverines to a final record of 18-7 (7-3 Region) during the 2018-2019 season. Prince Avenue wishes Coach Roberts all the best as he transitions from the field of education into the business world.
