On behalf of Volunteers of America Chesapeake & Carolinas (VOACC), AmeriHealth Caritas DC, Quantified Ventures, Pathways to Housing and Unity Health Care, we are excited to invite you to a special webinar featuring the recently launched Hope Has A Home program. Over time, this innovative program will drastically expand the number of medical respite beds and comprehensive support available to individuals experiencing homelessness and suffering from chronic illnesses in the District of Columbia.

Hope Has A Home is a 16-bed program with medical respite, temporary housing, behavioral health and social support services for individuals experiencing homelessness in the District of Columbia. It has been designed to improve the health status of participants and connect them to more stable housing while simultaneously reducing healthcare spending for the participating Medicaid population. The program addresses the complex medical, behavioral and social needs of participants, and focuses on their long-term social reengagement.

During the webinar, you will learn more about Hope Has A Home's unique model and financing approach, engage with our partners from the public and private sectors who helped to make the program possible, and hear from individuals that have been served by the program.
MODERATOR
Stephen Samuels
Stephen is Vice-President of Innovation & Impact Investing at Volunteers of America. In that role, he helps to lead the piloting and scaling of new business ventures across the national organization, and match those opportunities with impact investor partnerships. He has been a leader in community development and affordable housing finance for over 10 years, following a marketing and business development career in the for-profit sector. He is a certified urban planner with the American Institute of Certified Planners.
PANELISTS
Rosalyn Carr Stephens, RN, MSN, CCM
Rosalyn has served as Market Clinical Director of Population Health Management at AmeriHealth Caritas DC since 2017. With expertise in operational analysis, population management strategies and clinical staff training, she has over 35 years of clinical and health insurance industry experience in Florida, Maryland, New York, Virginia, and the District of Columbia. 
Jana Berhow, LICSW
Jana is a Licensed Independent Clinical Social Worker with over 20 years of experience in public and private settings serving the behavioral health and human services needs of the residents of the District of Columbia. As Vice President of DC Programs, Jana lead teams in VOACC's behavioral health, intellectual/developmental disabilities program, and Hope Has A Home programs.
Dr. Catherine Crosland
For the past ten years she has worked for Unity Health Care as Medical Director for Homeless Outreach Services and Director of Homeless Outreach Development. In addition to providing clinical care for people experiencing homelessness in a variety of clinical settings ranging from shelter based clinics to street outreach medicine, she also manages over ten homeless outreach clinics. 
Alison Rein
As Vice President of the Quantified Ventures Health and Human Services team, Alison works to mobilize new sources of capital to advance creative, cross-sector, and collaborative solutions to thorny issues in public and population health. With 25 years of experience in the strategic consulting and healthcare fields, Alison excels at partnership development, sustainable financing, program design, and implementation planning.
