The Milwaukee Heart Health Learning Collaborative is a partnership between the WI DHS Chronic Disease Prevention Program (CDPP), Health Connections, Inc., and Y-EAT Right. As co-leads, we are committed to bringing together community and clinical stakeholders to strategically leverage community assets and services to improve cardiovascular health in Milwaukee.
Our goal is to engage and reach community members in familiar places such as places of worship, community centers, housing complexes, and various service establishments. We are inviting you to join us for our first Community Partner Kick-off Meeting.

During this event, you will have the opportunity to:
- Learn about the major goals and actions of the new CDC Grant dedicated to impacting cardiovascular health.
- Understand how the voice of the community will play a central role in the actions undertaken by the Learning Collaborative.
- Discover the extent to which your input will be sought throughout the collaborative.
- Review the general topics that will be addressed over the course of the 5-year collaborative.
- Explore opportunities to actively participate in quality improvement program development, committees, and evaluation strategies.
