Chapter Presidents: 

Please complete the below Chapter Information Form indicating officers for the  upcoming year July 1, 2019 - June 30, 2020,  and return by July 7 to cthomas@nccualumni.org or mail to the address below or fax to 919-560-5864.

Every chapter must complete a form, even if the officers remain the same. Remember all chapter officers should be financial both nationally and locally.
 
As a reminder, annual chapter assessments of $100 for FY2020 are due by July 1, 2019. If you have not paid your FY2019 chapter assessment, please send with your FY2020 assessment.
 
A Statement of Income and Expenditure form is also required to be completed on or before July 10, 2019 . Remember to attach a copy of your June 30, 2019, bank statement(s). Our Auditor need these items completed and received by the due dates to provide us with our audit in a timely manner.
 
It is suggested that you share this document with your Chapter Secretary and Treasurer for completion by the deadline. All Treasurers should send forms directly to Chevella Thomas at cthomas@nccualumni.org. All assessments, dues, and other forms /documents may be mailed to the address below.
