additional covered benefits!

Fulcrum is seeking licensed acupuncturists to join our AcuNet network and qualified massage therapists to join our TruTouch network.

Conservative care is gaining consumer demand because of efficacy in pain management. Due to this demand, Fulcrum is working with insurance plans that have interest in expanding this benefit to Medicare Advantage members. To meet this need, Fulcrum is currently seeking qualified massage therapists and licensed acupuncturists to join our network for providing health plan covered services. 

If you’re currently in an active massage therapy and/or acupuncture practice, we encourage you to consider applying.

  • No fee to join, no membership dues, no annual costs to participate
  • Your name and clinic will be listed in the directories and websites of Fulcrum's clients
  • Professional development through continuing education classes
  • Receive referrals from within ChiroCare, Fulcrum's chiropractic network



Incentive Opportunity
We are now offering an incentive for eligible clinics in the A and CCoE assigned tiers based on clinical outcome submissions. In addition to the financial incentive, submitting your clinical outcomes shows how you compare to your peers and generates shared learning from colleagues. More information on the incentive and the eligibility guidelines are available in the Learning Hub under the Incentive tab.
COMPLIANCE
Disclosure of Ownership and Cultural Competency Requirement
Fulcrum asks all providers to complete the Disclosure of Ownership form each year. This form is related to disclosure of ownership, business transactions and excluded providers.
The Disclosure of Ownership is a requirement set forth by the Code of Federal Regulations ( 42 CFR Part §455 ) and from the Minnesota Department of Human Services (DHS) and the Centers for Medicare and Medicaid (CMS). These entities require all health plans and their delegates, including Fulcrum, to ensure that its network providers submit documentation.

While we ask for this information annually, there are additional times you may need in order to complete this information such as:
  • upon initial contract with Fulcrum
  • when your information changes and/or upon contract renewal.

It is important to remember that if there are any changes to the information listed on your Disclosure of Ownership form, you must complete an updated form within 35 days. While Fulcrum asks you to complete the Fulcrum Disclosure of Ownership Form, you are able to submit your disclosure form used for other health insurers by fax at 763-204-8544 so long as identical information is requested.

For your convience this form is electronic and takes approximately 10-15 minutes to complete. Fulcrum is requesting that each clinic complete the online form by November 29, 2019.

This online form is located on a secure, encrypted webpage.

If you know or suspect fraud, waste and/or abuse is occurring in your place of business please report it immediately to Fulcrum’s compliance hotline at 1-866-714-0526. Tips can be left on this voicemail 24/7 by anyone (provider, office support staff, patients, etc.).

We value your opinion. The feedback we gather from this annual survey helps identify areas of interest and of concern to the ChiroCare by Fulcrum Health network. We also use your responses to assist in the development of new programs, supporting materials, and educational opportunities.

It is vital that you report any practice changes to ensure that we have your current address, phone, fax, and email address. Any changes or corrections to your TIN/name combination should be communicated immediately. The Provider Update tool within ChiroCare Connect offers a quick and electronic means to report changes. Simply select the “Billing Information Update” option within the Provider Update tool.
