A BIG MONTH FOR ESPERANZA HEALTH CENTER
GOVERNOR WOLF AND SENATOR TARTAGLIONE
AT EHC - KENSINGTON
Earlier this month, we had the privilege of hosting Governor Tom Wolf, Senator Christine Tartaglione (PA-2), and other state and community leaders at our Kensington site for a press conference to discuss the growing opioid crisis and its devastating effects on the surrounding community. We also heard EHC's Chief Executive Officer, Susan Post, share her appreciation for other Kensington organizations and advocates and their commitment to continue the fight for our community.
SENATOR SHARIF STREET AT EHC - HUNTING PARK
On Wednesday, September 15, 2021, Senator Sharif Street (PA-3), along with former Mayor John Street and other community leaders, held a press conference at our Hunting Park site to address the low vaccination rates among predominantly black and brown communities in Philadelphia.
2021 COMMUNITY HEALTH QUALITY RECOGNITION BADGES
We are proud to announce that Esperanza Health Center was the recipient of five 2021 Community Health Quality Recognition (CHQR) badges presented by the Health Resources & Services Administration (HRSA). These badges recognize Health Center Program awardees that have made notable quality improvement achievements in the areas of access, quality, health equity, and health information technology for the most recent Uniform Data System (UDS) reporting period. Additionally, COVID-19 badges were introduced to recognize health centers’ contributions to the public health emergency response in the areas of data collection, testing, and vaccinations. EHC's badges include the following:
Health Center Quality Leader - Silver
Health Center Quality Leaders achieve the best overall clinical quality measure (CQM) performance among all health centers. They are recognized in the following tiers: Gold (top 10%), Silver (top 11-20%), or Bronze top (21-30%).
Advancing Health Information Technology (HIT) for Quality
Recognizes health centers that meet all criteria to optimize HIT services that advance telehealth, patient engagement, interoperability, and collection of social determinants of health to increase access to care and advance quality of care.
COVID-19 Data Reporter
Recognizes health centers that supported data for public health emergencies with response rates of 90% or more to the weekly Health Center COVID-19 Survey from April 10, 2020 to July 2, 2021.
COVID-19 Testing
Recognizes health centers that tested more than 50% of their 2020 reported UDS patient populations from April 10, 2020 to July 2, 2021 and have a response rate of at least 50% to the weekly Health Center COVID-19 Survey.
Patient Centered Medical Home Recognition (PCMH):
Recognizes health centers with PCMH recognition in one or more delivery sites.

DELKA CASTELLANOS
Last week, EHC's Social Services Supervisor, Delka Castellanos, was officially sworn in as a United States Citizen! According to Delka, she is "blessed to become a U.S citizen" and is excited to "participate fully in [her] responsibilities as a citizen, including voting." We are so grateful to celebrate this big accomplishment alongside Delka and give thanks to God for bringing her to Esperanza Health Center!
