FILING YOUR 1095-C FORMS

The IRS requires Applicable Large Employers (ALE) to file 1095-C Forms each year. Employers may file the 1095-C forms themselves or contract directly with a third-party vendor to file these IRS-required reports for the 2021 tax year by March 2, 2022. In some cases, your payroll vendor may even offer the service to complete this process for you (ex. ADP).

TML Health will continue to file the 1095-B reports, which are a requirement for health plans and insurers.

WHAT ASSISTANCE CAN TML HEALTH PROVIDE?

TML Health can assist our member groups by providing data to show who was covered under your group health plan each month. We have uploaded a spreadsheet report that you can use as a starting point for 1095-C reporting. This report shows employees and dependents that were covered under a TML Health medical plan during the reporting year.

Employers are responsible for verifying this information, as well as adding any additional eligible employees or dependents that are not enrolled in your group health coverage.

TO FIND THIS REPORT:

1. Go to tmlhealthbenefits.org and select Login as a Benefit Coordinator. Enter your credentials to sign onto TML Health Online.

2. From TML Health Online’s home page, select the Benefit Center tab and click on Reporting Panel.

3. From here, you can log into the TML Health Portal using your work account.

4. Once inside the TML Health Portal, click on the Regulatory Reporting tab to access your Group Assist spreadsheet.
