Earn Credit for Attending
Attendance for the Town Hall Gathering will meet a portion of the Program Engagement CI requirement for 2026. In order to receive credit for attending, you must complete the necessary information when joining via Zoom.
Attendees will also have the opportunity to earn 1.25 CME credits for attending this event. To receive AMA PRA Credits and/or a certificate of attendance, attendees must have an active account with Cloud CME and complete the attendance verification during the meeting.

| | Review Your 2025 Clinical Integration Index Score Report | | |
Your 2025 Clinical Integration (CI) Index Score Report is now available and summarizes your performance in the 2025 CI Program.
Please review the communication (adult medicine PCPs | pediatricians) we sent on August 19 for details on where to find your report in your practice’s OneDrive account, as well as instructions for submitting supporting documentation if you choose to participate in the supplemental file submission process. All submitted materials for both Adult PCPs and pediatricians are due by Wednesday, September 9.

As we move into the second half of year, now is the perfect time to review documentation specificity and ensure chronic conditions have been addressed and documented to accurately reflect patient complexity.
Documentation Focus Areas
| | |
Major Depression
- Document and code to the highest specificity
- Specify whether the condition is a single episode or recurrent
- Indicate severity as mild, moderate, or severe
- Document partial or full remission, when applicable
Tip: Avoid documenting only “Depression.” Instead, include the highest level of specificity to accurately capture the patient’s condition.

Chronic Kidney Disease (CKD)
- Document the stage of CKD
- For Stage 3 CKD, specify whether it is Stage 3a or Stage 3b
- Include the most recent eGFR, when clinically appropriate

Obesity
- Severe (Morbid) Obesity: BMI 35.39.9 with comorbidity (DM, HTN, sleep apnea, etc.)
- Morbid Obesity: BMI ≥40 (regardless of comorbidities)
Dementia
- Document the underlying cause (Alzheimer’s disease, vascular dementia, Lewy body dementia, etc.)
- Specify the severity (mild, moderate, severe)
- Note any behavioral disturbances (agitation, psychosis, mood changes, sleep disturbance)
Tip: Avoid “unspecified dementia” if more detail is available.
Heart Failure
- Acuity: Indicate whether the heart failure is acute, chronic, acute on chronic, or end-stage
- Type: Specify whether systolic, diastolic, or combined heart failure
- Specify the stage (A, B, C, or D) and class of HF (I, II, III, IV), if known
Diabetes Mellitus
Best documentation of Diabetes Mellitus includes:
- Type: type 1, type 2, type 1.5, secondary, or drug-induced
- Complications: CKD, neuropathy, nephropathy, PVD, retinopathy, non-pressure ulcers
Tip: Documentation should be present to support associated complications along with documentation for DM.

The Practice Communication requirement and the HIPAA Privacy Training requirement of the 2026 CI Program are due Wednesday, September 30. These requirements must be completed to remain an active member of our Clinically Integrated Network.
If you have not done so already, here’s how to complete each requirement:
Practice Communication
Log into the MSHP Provider Portal and click on the Practice Profile tile to get started. For further instructions, please review the Provider Training Guide or the Office Manager Training Guide.
HIPAA Privacy Training
Click here and sign in to directly access the HIPAA: Privacy and Security MSHP Education Program 2026. Once you completed the course, submit your attestation and you will receive your course completion certificate. Please save this certificate for your records.
All 2026 CI Program requirements must be completed by the established deadlines for PCPs to remain eligible to receive care coordination payments and/or the annual incentive payment. A base care coordination payment is issued to PCPs on a quarterly basis for all four quarters, with the annual incentive/ reconciliation payment being a separate payment.

With the new school year approaching, you may be seeing an increase in well child visits, physicals, and routine vaccinations.
Executive Order 14420, signed on August 10, recommends reducing and spacing out childhood vaccines. The American Association of Pediatrics (AAP), however, continues to recommend vaccines to protect against all 18 diseases in its 2026 immunization schedule.
In the AAP Statement in Response to Executive Order on Vaccines, AAP President Andrew D. Racine, MD, PhD, FAAP, affirms that "there is no new evidence to justify significant changes to childhood immunization guidance."
Should your patients or their parents have questions about routine childhood vaccinations, you may consider directing them to this FAQ. The AAP also offers fact-checked articles on links between vaccines and autism, receiving multiple vaccines, US vaccine recommendations compared to other counties, and other common concerns and talking points.
This order comes as the number of confirmed measles cases in the United States are at their highest since 1991, with about 69% of cases occurring in children and teenagers. The AAP continues to recommend a two-dose series of the MMR vaccine to protect patients.

Medical Aid in Dying:
A Decade of Advocacy and Dr. Jeremy Boal's Influence

Effective August 5, 2026, Medical Aid in Dying is available to terminally ill New Yorkers with less than six months to live under Public Health Law Article 28-F. The law expands end-of-life options for terminally ill New York residents and affirms their right to make informed decisions to reduce suffering at the end of life.
Dr. Jeremy Boal, past chair of the MSHP Board of Managers and former Chief Clinical Officer of the Health System, was instrumental in getting this legislation passed. New York Magazine published a poignant piece on his work and advocacy.
