On our patient portal, you selected that you forgot your login and you were prompted to create a new login and password. If you did not call the office to receive the new activation code, it was mailed to you. You are receiving this email because you have not logged in since changing your login and password on the patient portal at CNY Family Care. As a result, we have attached a one-time activation code that will be required to log in successfully.
Our practice is asking that all patients activate and utilize their patient portal account. The most efficient method for your physician to provide you with your test results is through our patient portal. There are many benefits to using the patient portal. If you have already reactivated your account, please disregard this email. However, if you have not, follow the directions below to reactivate your account today.

For instructions on how to be correctly set up on this patient’s portal account as an Authorized Representative, please call our office at 315-463-1600. Any accounts found to be operated by someone other than the patient will be reset.
