On our patient portal, you selected that you forgot your login and you were prompted to create a new login and password. If you did not call the office to receive the new activation code, it was mailed to you. You are receiving this email because you have not logged in since changing your login and password on the patient portal at CNY Family Care. As a result, we have attached a one-time activation code that will be required to log in successfully.


Our practice is asking that all patients activate and utilize their patient portal account. The most efficient method for your physician to provide you with your test results is through our patient portal. There are many benefits to using the patient portal. If you have already reactivated your account, please disregard this email. However, if you have not, follow the directions below to reactivate your account today.

 

Please note, if you have received this email and are not the patient listed above, please disregard this email. For instructions on how to be correctly set up on this patient’s portal account as an Authorized Representative, please call our office at 315-463-1600. Any accounts found to be operated by someone other than the patient will be reset.

 

The patient portal provides many benefits that include:

- Eliminates long hold times when calling your medical provider and team in the office.

- Provides the capability to request medication refills.

- Provides the ability to request an appointment.

- Allows communication between you and your medical provider.

- Provides the ability to view all upcoming and past appointments.

- View/Update your past medical, family, and social history.

- View/Update your account, insurance, and pharmacy information.

- View/Print your medical records/test results using the chart tab.

- Make payments on balances.

Directions:

1.  Go to our website, www.cnyfamilycare.org; at the top of the web page, click the button for the patient portal.

2. Enter the login name you created on the portal "Sign in." (If you can't remember your login name, please call our Patient Portal Representative at 315-748-7869, and they will provide this to you.)

3. Enter your password & Click Sign-In

4. Answer a security question to verify your identity.

5. Next, you will be prompted to enter an activation code. Enter your portal activation code as it is shown below.


Your activation code is not your password.

Note: This code is case-sensitive and must be entered exactly as it appears above.


If after receiving this email, you change your login and password again on our patient portal, the activation code listed above will no longer be valid. You will need to call the office to receive that new code.


Additionally, please remember that the Patient Portal is to discuss YOUR non-urgent medical needs and information ONLY. If you must discuss someone else, please use their portal or contact the office at 315-463-1600.
