One Reminder at a Time:
Care Beyond the Clinic
| | Through our Enhanced Care Management (ECM) program, clients like Manuela are receiving support that goes beyond appointments, helping improve her everyday well-being. Through the ECM program, Manuela now receives assistance with scheduling and appointment reminders. This, combined, helps her stay on top of her health, something she previously struggled with on her own. | | With our Upland location being embedded in her community, she received assistance with reminders for her doctor appointments and a monthly grocery allowance to help her eat healthier, which matters deeply given her family history of diabetes and high blood pressure. | Our ECM program reflects our commitment to meeting members where they are, supporting not just medical needs, but the everyday barriers that affect long-term health outcomes. Scroll below to our Podcast with our Board Member and Kaiser Permanente's Doctor Tim Jenkins on social determinants and its imapct on the Inland Empire. | | Your donation is the help our Inland Empire community members need. Stories like Manuela's are a reminder that personalized assistance, nourishment, and grace can change everything. Donate today! | |
What Can Our Healthcare Initiative
Do for You?
| | Getting health care can feel overwhelming, especially when cost, language, transportation, or navigating the system create barriers. Through our Health Care Access Initiative, we help individuals and families connect to the care and support they need to improve their health and well-being. | | |
Our services include support with accessing primary and preventive care, care coordination, health education, referrals to community resources, and assistance navigating health systems and benefits. We also help address barriers that can make it harder to stay healthy, including access to nutritious food, transportation support, and connections to additional community-based services.
Whether someone needs help finding care, understanding their options, or staying connected to important services, our team is here to provide support every step of the way.
| | All of these services work together to support our community members, whether you need medi-Cal or help planning getting to appointments. To get started, visit our website or stop by a Community Resource Center near you to learn more. | | During Volunteer Appreciation Month, we are proud to celebrate Marilyn Akins, recently recognized by Catholic Charities USA as a finalist for the 2026 Volunteer of the Year Award. For more than 20 years, Marilyn has faithfully served neighbors facing hunger as lead volunteer for our community-based food distribution efforts, helping coordinate more than 100 volunteers in support of seniors, families, and farmworkers across our region. | | We extend our heartfelt gratitude to Marilyn, to all of our dedicated volunteers, and to our Board of Directors, whose service and leadership help advance our mission throughout the community. | |
Healthy Communities
Start With Connection
| | In this episode of Community in Focus, CEO Tim Ney sits down with board member Dr. Tim Jenkins of Kaiser Permanente for a thoughtful conversation about health, service, and the well-being of our region. Dr. Jenkins shares his personal journey, reflects on the social factors that shape health outcomes, and discusses why expanding access to care in the Inland Empire matters. It’s an inspiring conversation about leadership, community partnerships, and building healthier futures together. | |
Our counseling service providers are top-tier and provide care that matches. We were delighted that our very own Jeniffer Sasse, MSW Intern Supervisor, was recognized by the Phi Alpha Honor Society with a complimentary membership in appreciation of her work preparing MSW students for the social work profession. This recognition reflects the value of investing in future social workers and strengthening the next generation of community-centered leaders. | |
Grateful for
Our Community Partners
| | As we highlight the importance of healthcare access this month, we are grateful for the support that helps address health beyond the doctor’s office. Through this generous grant, Catholic Charities is expanding access to nutrition security programs like CalFresh and WIC by providing enrollment assistance, benefits navigation, nutrition education, and community outreach across San Bernardino and Riverside Counties. This support will help connect hundreds of individuals and families to essential food resources while reaching thousands more through outreach and education, advancing healthier communities and reinforcing the connection between nutrition, prevention, and overall well-being. | | Ready to see the strategy behind the impact? We invite businesses and community leaders to join the tour. Schedule your Program Tour today, and let's explore how your leadership can amplify our mission and transform our community together. | | Support for Every Step Forward | | |
