Committee on Tactical Combat Casualty Care: Meeting Minutes

1. Chairman's Welcome: Dr. Frank Butler, the Chairman of the Committee on Tactical Combat Casualty Care (CoTCCC), convened the meeting. After a review of the agenda, there were no conflicts of interest reported by the attendees. Dr. Butler gave a brief overview of the CoTCCC and Joint Trauma System (JTS) organization and the standard procedures of the CoTCCC for updating the TCCC guidelines. Dr. Butler presented TCCC certificates of appreciation to CoTCCC members COL Samual Sauer and CMDCM Steve Viola, who are retiring from active service. He also presented a TCCC Special Award to COL (Retired) John Kragh and noted that, without Dr. Kragh's landmark research on tourniquet use in Iraq, "We would still be arguing about whether or not to use tourniquets, rather than discussing how to use them most effectively."

2. Combat Medic Presentation: SSG Ernest Parish and SSG Myles McKenzie (medics from the 75th Ranger Regiment) and LTC David King (surgeon) discussed the care of two casualties on a Special Operations mission in southeastern Afghanistan. Insertion was by helicopter and the landing zone was in difficult terrain. The enemy lay in wait in the target building. Ambient temperatures were 35-40º F, so all casualties had 3-4 layers of clothes on, complicating their care. Six men were wounded on this mission. The comments that follow apply to the first two casualties. The first casualty was wounded by enemy fire from portholes in the walls as the team approached the target. He was moved to a secondary casualty collection point 50-75 meters further away from the target.

ABSTRACT

This is the first of a two-part article discussing loadcarriage- related paresthesias, including brachial plexus lesions (rucksack palsy), digitalgia paresthetica, and meralgia paresthetica. Paresthesias are sensations of numbness, burning, and/or tingling, usually experienced as a result of nerve injury, compression, traction, or irritation. Rucksack palsy is a traction or compression injury to the brachial plexus, caused by the shoulder straps of the rucksack. The patient presents with paresthesia, paralysis, cramping with pain, and muscle weakness of the upper limb. Muscle-strength losses appear to be greater in those carrying heavier loads. Hypothetical risk factors for rucksack palsy include improper load distribution, longer carriage distances, and load weight. Nerve traction, compression, and symptoms may be reduced by use of a rucksack hip belt; wider, better-padded, and proper adjustment of the shoulder straps; reduction of weight in the rucksack; a more symmetric distribution of the load; and resistance training to improve the strength and hypertrophy of the shoulder muscles.

ABSTRACT

Topical burns from the use of garlic have been reported rarely in the medical literature. Most cases have resulted from the use of naturopathic or home remedy treatments. A 20-year-old male military Servicemember presented to a military wound care clinic 7 days after applying a homemade topical preparation of garlic to the zygomatic region of the right side of his face. The patient had consulted the Internet for treatment of a minor skin lesion in that area. He created a garlic paste, applied it to the affected area, and covered it with a dressing. Twelve hours later, he noted an intense burning sensation where he had applied the garlic paste.

A Letter from the 2017 MHSRS Planning Committee
                                                                                                                                  
The MHSRS is the Department of Defense's premier scientific meeting. This Joint symposium provides a collaborative environment for military medical care providers with deployment experience, DoD scientists, academia, and industry to exchange information on research advancements and health care developments in the areas of Combat Casualty Care, Military Operational Medicine, Clinical and Rehabilitative Medicine, and Military Infectious Disease Research Programs. It is the only scientific meeting that addresses the unique medical needs of the Warfighter. 

The location and date of the 2017 MHSRS are still to be determined. This information will be posted on the MHSRS website and communicated via a mass e-mail when it is available. PLEASE DO NOT EMAIL US asking this question. 

The 2016 MHSRS broke all previous records for abstract submission and attendance! The 2017 MHSRS will offer more than 60 scientific breakout sessions that includes new topics such as Updates in Prolonged Field Care and Knowledge Translation as well as more in-depth topic discussions in the form of tract sessions. New tract sessions for 2017 include Global Health Engagement and Environmental Exposure Monitoring. In addition, the 2017 MHSRS will continue to highlight the research contributions of Young Investigators with a dedicated plenary session. 

The MHSRS website ( https://mhsrs.amedd.army.mil/SitePages/Home.aspx ) will be accepting abstracts from January 17, 2017 through March 6, 2017. 

Submitters will be notified of results by 8 May 2017.

Transforming Anger: Proven Strategies

During deployments, Servicemembers may use anger as a normal and healthy coping skill in combat situations. But anger can sometimes negatively and uncontrollably carry over into relationships with family and friends after returning home. This webinar will offer health care providers tools to recognize the dysfunctional cognitive processes that trigger anger behavior. Presenters will discuss proven strategies to transform the negative emotional, cognitive and physiological aspects of anger, into more constructive behaviors. 

The webinar will feature tips focused on maintaining physical health to reduce and ward off vulnerability to anger. National Tactical Medicine Competition  

S OARescue, in partnership with multiple organizations, officially announces the National Tactical Medicine Competition (NTMC). This competition is being held on Sunday, May 21st in Charlotte, NC at the Charlotte-Mecklenburg Fire and Police Training Academy (1770 Shopton Rd, Charlotte, NC 28217). Registration for competitors is open We have (1) of the (15) spots left, we will then begin a waiting list.
 
The National Tactical Medicine Competition is an event created to allow tactical medical providers from around the country the opportunity to collaborate and compete in a community of peers. Civilian Tactical Medicine is a relatively new discipline within the world of pre-hospital medicine. Together with The Committee for Tactical Emergency Casualty Care, we are creating opportunities to increase awareness, collaboration and training. This competition is a great opportunity to bring like-minded individuals together and support a cause greater than self.
 
The National Tactical Medicine Competition is based on the Tactical Emergency Casualty Care guidelines and all simulated patient care will be inline with the currently approved guidelines. The Committee for Tactical Emergency Casualty Care (C-TECC.org) endorses the NTMC. 
 
Proceeds from the Competition will benefit the equipping and training of first responders, with the skills and equipment needed to save lives.
 
Teams of two competitors will negotiate through a series of events to challenge both clinical and physical abilities. While the "lanes", as they are called, will be agnostic to provider level, they will require competency and skill to negotiate successfully. Competitors will proceed through the phases of care. Beginning at direct threat care, minimal treatment is performed but physical ability will be required to move the patient. Teams will transition to indirect threat care where they will be expected to further treat the patient and prepare for evacuation. Following, teams will evacuate the patient to a given location and continue to provide care during evacuation. Teams will then have to continue through specialized skill sets found only in tactical medicine.
 
The Mission of the Competition is simple, To Preserve Life. Through friendly competition among peers in a community of professionals, competitors and observers alike will all be able to take something away from this unique event.
