Community Health Worker: Proving Value and Finding Sustainability in Primary Care

Practice Transformation Institute (PTI) recently participated in the Michigan HIMSS 2025 Fall Conference - Healthcare on the Move: Michigan’s Roadmap to a Healthier Tomorrow.

 

The HIMSS Michigan Chapter, a mission-driven nonprofit, offers unique experts in health innovation, public policy, workforce development, research, and analytics to support leaders and organizations across the healthcare ecosystem. Through a community-centric approach, HIMSS delivers insights, education, and events that help providers, payers, governments, startups, life science, and other health organizations make informed decisions and drive meaningful changes.


The HIMSS Michigan Chapter 2025 Fall Conference brought together professionals from across Michigan for two days of valuable learning and discussion on topics such as:

  • Community Information Exchange
  • Cybersecurity in Healthcare
  • Data Exchange between POs and Payers
  • Driving Transformational Change
  • Enterprise Virtual Care Transformation
  • Integrating Community-based Organizations and CHWs into the SDOH Workflow

PTI shared insights on the importance of Community Health Worker (CHW) sustainability with a major focus on health plan billing requirements for CHW services, and sustainable models of care, in primary care. PTI remains committed to helping clarify strategies that support CHWs, particularly in the areas of CHW core roles, certification, ongoing education requirements, coding/billing and embedment n a primary care clinic.

 

A key question raised during the session was: “Is there a single process for CHW coding, billing and reimbursement across all health plans?” The answer is no! Each health plan, including Medicare has unique requirements for CHW enrollment, ICD-10 selection, CPT, and HCPCS codes reimbursement.

 

To address these differences, PTI is currently offering a two-part educational series designed to provide clarity on the billing and reimbursement process for CHW services.

The program will cover how to:



  • Become a Certified CHW
  • Explain billing requirements for Michigan’s payers including Medicaid and Medicare
  • Choose HCPCS and CPT codes to report services provided by a CHW
  • Apply ICD-10-CM Z-codes to report social determinants of health
  • Discuss potential functions and workflows in organizational settings
  • Identify documentation requirements

Michigan LARA Requirements for Osteopathic Physicians


MI LARA License Renewal Cycle: 3-years


150 hours - A minimum of 60 hours must be earned through MI LARA Category 1 programs, and 40 hours must be AOA approved. 


 Implicit Bias Training: 3 hours per cycle

Medical Ethics: 1 hour per cycle

Pain and Symptom Management: 3 hours per cycle

(1 hour must include controlled substance prescribing)
