The Michigan State University College of Osteopathic Medicine has achieved an important milestone, becoming the first osteopathic medical school to be included in the prestigious National Institutes of Health Medical Scientist Training Program, or MSTP.

Beginning in late July, Blue Cross Blue Shield of Michigan and Blue Care Network patients with Medicare Advantage plans may receive the annual Health Outcomes Survey, or HOS, conducted by the Centers for Medicare & Medicaid Services. So now is the time to discuss HOS topics with your patients. Provider-patient communication can lead to better health outcomes.


The following are HOS topics for 2025:


  • Risk of falling
  • Incontinence
  • Physical activity
  • Improving or maintaining physical health
  • Improving or maintaining mental health


Blue Cross and BCN are encouraging at-risk members to initiate conversations with a provider about incontinence and fall prevention. However, these conversations can sometimes be embarrassing or tough for members to bring up or they may forget to discuss if it’s something that only happens once in a while. For this reason, providers are encouraged to help “break the ice” and make it a regular part of their discussions.

Care Teams Enhance Transition of Care to Reduce Readmissions


Every hospital readmission is more than just statistics and data – it disrupts lives, renews or worsens stress for patients, families and caregivers, and signals missed chances for better continuity of care. According to CMS (2024), thirty-day readmission rates are a key quality measure, often reflecting “quality of hospital care, poor discharge planning, and ineffective coordination of post-discharge services.” Evidence shows that reducing preventable readmissions requires multiple interventions. These interventions include patient and caregiver education, discharge checklists, medication reconciliation and review, post-discharge follow-up, and identifying and addressing social determinants/drivers of health (SDOH) and disparities (CMS, 2024).

The period after discharge, or transition of care (TOC), is a critical time for these interventions. Patients may leave hospitals, skilled nursing facilities or other settings with new prescription medications, complex instructions, unanswered questions, and unmet social needs, not to mention limited abilities. Missed follow-up medical visits, confusion about care plans, appropriate use of medications, or unaddressed social needs often lead to readmissions. Care team members, especially Community Health Workers (CHWs) play a vital role in bridging these gaps. By leveraging shared culture and language CHWs build trust, address health literacy, simplify instructions, coordinate follow-up appointments, provide phone or home check-ins, and connect patients to much needed SDOH resources such as housing, food, utility support or transportation.


Preventing readmissions is not just a medical process – it is a human one. CHWs and other care team members provide health education and coaching, empowering patients to actively manage their healthcare conditions and adopt healthy behaviors.


CHWs support individuals where they live, work, and recover, helping ensure safe transitions, reducing avoidable hospital stays, and providing the connection and support patients and families need. Integration of CHWs into care transition offers a cost-effective and patient-centered approach to improving health outcomes and reducing healthcare utilization, particularly for vulnerable populations facing complex health and social challenges. Their unique ability to build trust, navigate complex systems, and address non-clinical needs makes them an invaluable asset in promoting successful care transitions and a more equitable healthcare system.

Michigan LARA Requirements for Osteopathic Physicians


MI LARA License Renewal Cycle: 3-years


150 hours - A minimum of 60 hours must be earned through MI LARA Category 1 programs, and 40 hours must be AOA approved.
