Thank you to all employees who have already completed their biometric screening or submitted an External Provider Form. Your commitment to your health is appreciated.

If you have not yet completed your screening, the deadline is approaching fast. To qualify for the 2026 health insurance premium discount, both you and your covered spouse (if applicable) must complete a biometric screening or submit the External Provider Form by Saturday, Aug. 31.

Schedule a Biometric Screening Appointment:

Call (888-830-6538) or visit your patient portal to schedule your biometric screening appointment with Marathon Health.

Submit the External Provider Form:

If you prefer to visit your own healthcare provider, submit the External Provider Form. You may drop it off at a Marathon Health clinic or email this signed form to wellness@marathon.health to receive credit. Credit will be awarded within 5 business days of submission.

LabCorp and Marathon Health will be on site offering convenient, 15-minute screenings to help you complete this requirement. Measurements will include height, weight, waist circumference, blood pressure, cholesterol, and blood sugar A1c.
