NYOI Members Parent/Guardian Information

Dear Boys & Girls Clubs of South San Luis Obispo Parents/Guardians

Our Club is taking part in the National Youth Outcome Initiative that will be used to track the wellbeing of members in Boys & Girls Clubs nationally.

Members will be asked to fill out a survey between June 10-21, 2024, at the Club in person.  The survey takes around 20 minutes to complete.

Members will not put their names on the survey, and individual responses will never be shared in any report of the results.

We would like all members of our Club to take part in this survey, but it is completely voluntary. Survey participants can skip any questions they do not wish to answer.

If you would like to see the survey, a copy is available at our Oceano Clubhouse, 1830 19^th Street, Oceano. If you are not able to stop by the Club, please contact Rebecca for specific survey information.

Parental opt-out forms are available with your Club Sites Unit Coordinator. Please return the Opt-Out forms by June 7, 2024.  If you wish for your youth member(s) to complete the survey, no action is needed from you at this time.

Arroyo Grande tweens and teens will groove to the Zulu DJ Program, while Grover Beach and Oceano seniors dive into SLOMA's continuing art project. Let's make memories and think outside together! 🎨🎶
