Be Safe in the Sun | Sun and UV Exposure

Learn about some of the damaging effects that too much sun exposure can have on the skin. You'll also find tips to help you protect yourself and your family from getting too much sun.

Continuity of care is the consistent and coordinated delivery of healthcare services across time and places, so patients get consistent and high-quality care. While healthcare providers work hard to make sure there are no gaps in someone’s care, it is a team effort including you.

 

Transitions in your care is where you as a patient are most vulnerable. When you’re having a transition in your care, like moving from the hospital back to home, good continuity of care can:

  1. Lower your risk of going back to the hospital.
  2. Improves your satisfaction with your healthcare.
  3. Help you manage your chronic disease(s) better.
  4. Creates better communication between you and your healthcare providers.

 

Did you know that for every 5 patients leaving the hospital, 1 of them will have a problem after? This can happen for a few different reasons:

  1. Poor communication between healthcare providers.
  2. Your discharge instructions weren’t complete.
  3. There are errors with your medications.
  4. Little or no follow-up care.

 

It can seem like the responsibility only falls on your healthcare providers. But you as a patient have an important role to play. You are the most important part of your healthcare team! Here is what you can do:

  1. Follow your discharge instructions from the hospital.
  2. If your condition changes, tell your healthcare providers.
  3. Keep an updated list of your medications and allergies.
  4. Understand what your medications are for, why you are taking them, and when you need to take them.
  5. Go to your follow-up appointments.
  6. If you don’t understand something, ask questions!

 

Your healthcare doesn’t stop when you leave the hospital. The steps you take at home play a big role in your recovery!
