MEMBER PLAYER REGISTRATION PASS/DOCUMENT FOR PLAYER VERIFICATION/APPROVAL:

If an effort to ensure accurate verification for each player registration for USASA events in Sportlomo, please provide us with your current player registration pass/document for your organization. If you do not have a pass, please provide a receipt that each player receives upon membership.

• We will also be adding a share your content option to the website for future photo/video uploads.

• Prohibited Conduct Policy – with your association name on policy
• One on One Policy – with your association name on policy
• Section on website how to report/mandatory reporting --> REPORT A CONCERN

Q: How do I cancel a COI?

A:

• Step 1: Select which holder needs to be cancelled
• Step 2: Click the “Assign” drop down arrow
• Step 3: Select Cancellation
• Step 4: Fill out date of cancellation and reason (example: 05/02/2024 reason: permission to use field.
• not given)
• Step 5: Issue
