The Center for Transition to Adult Health Care for Youth with Disabilities

is looking for Youth

to Join the Youth Development Initiative (YDI)



Transition to adulthood can be difficult. Where do you go for help? The Center for Transition to Adult Health Care for Youth with Disabilities would like to share this exciting new opportunity for youth with disabilities. This opportunity will allow youth (12-17) with intellectual and/or developmental disabilities to gain an understanding of healthcare transition and gain advocacy skills, from and with young adults with disabilities. Please read the information and share with others who may be interested.

 

  

We are Looking for Youth to Join the Youth Development Initiative (YDI).

 

What is the YDI: This group of youth with IDD's will receive training and mentoring to help them understand healthcare transition at an early age. They will be mentored by young adults who have learned leadership and advocacy skills.

 

Who Should Apply? Young people (ages 12 – 17) with intellectual and/or developmental disabilities who are ready to learn about healthcare transition, self-advocacy, and getting ready for adulthood? 

 

YDI members will:

        Learn from other young adults about health care transition. 

        Meet peers and make friends.

        Learn about being an advocate for themselves and others with disabilities.

 

About Our Center

The Center for Transition to Adult Health Care for Youth with Disabilities helps young people with intellectual and developmental disabilities and their caregivers to take control of their healthcareThis Center provides resources, information, and support for moving to adulthood.

 

 

Health Care Transition and Youth

Health care transition can begin at any age, but usually begins during the teenage years. The Center for Transition to Adult Health Care for Youth with Disabilities begins with young people and their ideas about health care transition. We want people with disabilities to live as independent and healthy lives as they choose.
