A Pharmacist's Guide to Taking Medications Safely

Dr. Antoinette Coe, an assistant professor of clinical pharmacy at the University of Michigan, studies how to improve medication use and safety. Research suggest that half of medications prescribed for people with chronic diseases might not be taken correctly.

Heart medications can be a source of medication-related problems particularly among older adults who are on multiple medications. The medications may have side effects such as dizziness or fatigue or need dosage adjustments requiring monitoring and communication with their health care team.


People taking medications should understand what each medication treats and how and when to take it, including whether to take it with food.


Questions to consider for the doctor:


  • What is this medication prescribed for?
  • What are potential side effects, and what should I do if they occur?
  • How long do I need to take this medication?
  • Is there anything I should avoid while taking this medication, such as food?


Questions to consider for the pharmacist:



  • How should I take this medication?
  • Does this medication interact with any of my other medications?
  • Are there any side effects I should be concerned about?
  • What should I do if I miss a dose?
  • Where should I store this medication: in the refrigerator or at room temperature?


How people and their caregivers can avoid medication errors:


  • Have an accurate and up-to-date list of all medications they take, including prescription medications, over-the-counter medicines, herbal medications and supplements, and share it with their health care team.
  • Fill all their medications at one pharmacy, if possible, so the pharmacist knows their medications and can check for drug interactions or duplications.
  • Ask their health care team questions if any medication instructions are not clear.
  • Store medications properly and keep them out of reach of children.
