You will land on an ordering page but you want to go to the top the narrow black bar along the top ... on the left the small link Create Account ... or on the right under the My Account dropdown choose Register .... Either one they will take you to a page and you will see all the packages and then you just go through the registering process.
On the step 2 you will see me as the sponsor (my username is brettgurney if you need it) ... and you will also see the option to choosing the leg you want
Choose Left Leg
That's all there is to it
6 characters at least for username and password .... No dashes in the SS # or phone numbers
