Following yesterday's session on depth of focus (DOF), we offer today's video where Dr. Karolinne Rocha presents a DOF study that she and her colleagues performed at MUSC. The study compares patients' true accommodation and pseudo accommodation as enabled by various IOL profiles using iTrace measurements. 

 
THINGS TO LOOK FOR IN THIS TEACHING:
VSOTF (Visual Strehl Optical Transfer Function) is much like Contrast, so the higher this metric is – the better the quality of vision. The DOF (Depth of Focus) is the width of the curve at half of it’s height. That shows how much range of useable vision a patient has.  These two are normally inversely related. If you increase DOF you normally do so at the cost of contrast. Optical complexity is part of the problem with using and recommending these new lens types, and for best results it is vital to understand this concept.
 
THINGS TO CONSIDER WHILE YOU ARE WATCHING:
  • In a world of mass customization, can you really afford to do 80% of your implants with ONE IOL?
  • Are you optically savvy and trying to understand what IOL manufacturers are doing with their lens designs? 
  • Do you know enough optics to really mix and match lens types as KOL’s suggest from the podium? 
  • The iTrace measures and maps complex optical issues and presents them in straight forward ways. The less you know about optics, the more you need to lean on the iTrace and some of it’s lesser known functions.
 
THINGS TO CONSIDER STUDYING IN YOUR PRACTICE:
  • What is the DOF of my favorite monofocal? 
  • To maximize DOF, what is the best refractive target for each lens (HINT: if you are attempting monovision, and a specific lens type provides 1.0 Diopter of DOF, the proper power target should be -.5 and -1.5)…
  • How can you target the fellow eye after the first eye is done by measuring DOF?
