Hy-Vee will be offering a flu vaccine clinic at Xavier High School on Tuesday, October 15 from 10:30 A.M. – Noon. If you wish to have your child participate,
please follow these steps:
- Complete the Informed Consent Form with your child, including your signature (parent/guardian) at the bottom of the form.
- Arrange for payment in one of two ways:
- Send $32 with your student in the form of cash or check (payable to Hy-Vee).
- Send proof of prescription insurance for your student. This can be their insurance card, or a copy of the front of their insurance card. Be sure the card contains the “RxBin” and “RxPCN” numbers
Most prescription insurance plans cover the cost of the flu vaccine in full. If you are unsure about whether your child’s insurance will cover the flu vaccine, please contact your insurance company or Nicole Johnson at 319-393-4480 or

Each flu clinic participant will receive a voucher good for a 20-cent Fuel Saver + Perks discount.
