Meet the Self-Direction Team

Jacqueline Hemmy

With ten years of experience in the field, I continue to feel genuinely passionate about the work we do. Throughout my career, I have had the opportunity to serve in a variety of roles, which has given me a well-rounded perspective and a deeper understanding of the needs of the individuals and families we support. In addition to my professional experience, I also understand the system from the viewpoint of a family member receiving services. This combined perspective strengthens my commitment to providing thoughtful, person-centered support.

One of the most rewarding parts of my work is watching the individuals we serve achieve their goals and celebrating the progress they make along the way. I am truly excited about the continued growth ahead—not only within our department, but across the entire agency.

Outside of work, I enjoy reading, binge-watching The Office, traveling frequently to Florida and Texas, and spending quality time with my family, friends, and beloved dogs. I also take great pride in spoiling my nieces and nephews whenever I can!

Sujin Song

I have been working in the OPWDD field for approximately five years, providing Family Support Services (FSS) and supporting families through the OPWDD eligibility, outreach, and evaluation processes. One of the most meaningful parts of my work has been supporting Asian families and helping them better understand the system so they can access the services and supports they need.

I began working as a Support Broker about three years ago and have been with PCCS for one year and two months. I am truly grateful to be part of such a supportive and collaborative team, and I appreciate the opportunity to continue growing while serving families through Self-Direction.

Born to Belong Campaign

Inspired by Bruce Springsteen's "Born to Run" at Person Centered Care Services (PCCS), we believe that everyone—disabled and non-disabled—is born to belong.

Belonging isn’t something you earn; it’s something every human being deserves. It’s the feeling of being seen, valued, and included just as you are.

Since our founding, PCCS has been working to build a world where that belief becomes reality—where people with disabilities can live, work, and thrive as equals in their communities.

Your donation makes a world of difference.

Belonging in Action: Executive Conversations

Belonging in Action is a quarterly service conversation series hosted by PCCS executive leadership. These sessions create a safe, welcoming space for people supported, family members, employees, brokers, community class vendors, and others connected to PCCS to engage in honest, open dialogue—sharing perspectives, asking questions, and collaborating on ways to strengthen services, supports, and community connections.
