This memo provides clarification on the scope of services included within Residential Habilitation Services and Supports (RHSS) under the HCBS Developmental Disabilities (DD) waiver. It also outlines appropriate processes for accessing Long-Term Home Health (LTHH) and Private Duty Nursing (PDN) services for individuals receiving 24- hour RHSS.

Operational Memo 25-042 - State Supported Living Services Program (State SLS) and Community First Choice Program (CFC)
Audience: Case Managers, Case Management Agencies, Providers, Health First Colorado Members, HCBS Waiver Members
The purpose of this Operational Memo is to inform members enrolled in the State General Fund Program State Supported Living Services (State SLS), Health First Colorado (Medicaid) Members, Case Managers, Case Management Agencies (CMAs), and Providers about the program updates occurring after implementation of the new Community First Choice (CFC) program effective on July 1, 2025. This memo will provide operational guidance for the upcoming implementation of the CFC program that requires State SLS program members to transition to appropriate services whenever applicable.

The purpose of this Informational Memo is to inform Home and Community-Based Services (HCBS) members, stakeholders, and providers that the 1915(c) HCBS waiver amendments and renewals submitted to the Centers for Medicare and Medicaid Services (CMS) on March 7, 2025, were approved.

✴️ LTSS CCM System Stabilization and Enhancement Dashboard Update
HCPF made significant progress in 2024 to stabilize the LTSS CCM System. As work continues, HCPF acknowledges remaining issues across the CCM System in three key areas including Streamlining Interfaces, Data Quality, and Reporting for Case Management Agencies. Work by HCPF’s contractors (Gainwell and AssureCare) is underway to stabilize the CCM System by remediating these remaining issues. A new integrated schedule to resolve the remaining issues was provided to HCPF in December 2024. In addition, work is underway to complete key initiatives to improve access to care for our members including new programs such as Community First Choice, Money Follows the Person, the new Colorado Single Assessment, as well as a number of other enhancements that were delayed as a result of stabilization issues.
On June 24, a new update related to this important work was published. The next update will occur on July 8 (every other Tuesday).

EVV Listening Sessions are informal opportunities for stakeholders to bring questions to HCPF about EVV implementation. No new policy or technical processes will be announced in these Listening Sessions. New information will be discussed in the EVV Stakeholder meeting as needed and published on this page: https://hcpf.colorado.gov/electronic-visit-verification-stakeholder-workgroup.

Participant-Directed Programs Policy Collaborative (PDPPC)
About the PDPPC
The PDPPC brings together Consumer-Directed Attendant Support Services (CDASS) and In-Home Support Services (IHSS) stakeholders and the Department of Health Care Policy and Financing (HCPF) to collaborate on policy and program-related topics.
Meeting Structure and Schedule
Meetings will begin at 1 p.m. and end no later than 2:30 p.m. The first month of each quarter will have an extended meeting from 1 p.m. to no later than 4 p.m. MT. Meetings will occur on the 4th Wednesday of the month from January through October. During November and December, they will occur on the 3rd Wednesday of the month. The meeting is recorded and posted publicly. The meeting chat is shared with attendees of the meeting. Reminder emails with meeting documents will be sent approximately two Mondays before the meeting. Stakeholders may request documents mailed to them through United States Postal Service by calling the Participant-Directed Program unit at 303-866-5638.
