Our work to make health care more accessible and affordable is advanced through the generosity of individuals and organizations who share our vision. With Giving Tuesday on December 3, we hope you will consider making a gift to support our work. Our Health Policy Action Fund is open to gifts of any amount and will provide start-up funds to support our Emerging Initiatives that will address some of the most pressing health care challenges.

 

Specifically, the Health Policy Action Fund enables Duke-Margolis to respond to emerging issues by pooling philanthropic funding to support policy analysis, development, and educational opportunities. This funding ensures that health policy is informed by science, analysis, and representative stakeholder voices.

Global Drug Manufacturing and the Supply Chain



Duke-Margolis has released a new white paper co-authored by researchers Thomas Roades, Cameron Joyce, Madi Cordle, Gerrit Hamre, Mark McClellan, and former intern Katherine Hamilton (’25). The authors discussed the benefits, risks, and challenges associated with onshoring drug manufacturing, examined existing international initiatives to advance drug security and reliability, and outlined complementary steps that can be taken to secure the global pharmaceutical supply chain. Strengthening Accountable Care Organizations


Postdoctoral Associate Kun Li, Duke-Margolis researchers Frank McStay and Rob Saunders, and former researcher Jonathan Gonzalez-Smith, co-authored a paper in JAMA Network Open that examined the relationship between the performance of Medicare Accountable Care Organizations and the participation of Federally Qualified Health Centers in those organizations. North Carolina Medicaid Expansion



Research Director Rebecca Whitaker and Core Faculty member Mike Pignone co-authored a paper in APJM Focus that studied the North Carolina Medicaid expansion population and their preventative care needs. They found that this population has significant unmet preventative care needs, which will be important for North Carolina to consider when approaching provider capacity improvements. Read more here.

ACO REACH



Duke-Margolis researchers Rachel Bonesteel, Sara Debab, Will Bleser, Frank McStay, Rob Saunders, and Mark McClellan co-authored an article in Health Affairs Forefront that focused on the Centers for Medicare & Medicaid Services’ Accountable Care Organization Realizing Equity, Access, and Community Health (ACO REACH) model. This piece built on limited prior research of participant experiences and provides recommendations on how to improve and sustain the program.

Global Emergency Response and Preparedness


The Duke Global Health Innovation Center’s QuickStart Consortium has published their latest report to support emergency preparedness and response goals for emerging health challenges, such as the ongoing mpox outbreak. The report will be updated and shared every other week with their community—you can subscribe to their newsletter here.



Policy Impact

Social Return on Investment in Health Care


The Health Care Payment Learning & Action Network (HCPLAN) released their 2024 guidance document on “Value of Health Care Redefined: Social Return on Investment.” Research Director Will Bleser and Policy Research Assistant Sara Debab provided project support to members of CMS’ Health Care Fraud Prevention and Enforcement Action Team (HEAT) as part of the HCPLAN HEAT Operator team.

Primary Care in ACOs



The Primary Care Collaborative released their 2024 Evidence Report, “Primary Care: The MVP of MSSP,” which evaluated primary care in Medicare ACOs. Visiting Fellow David Muhlestein co-authored and Assistant Research Director Frank McStay reviewed the report, and both spoke at the release event for the report.

The demand for home-based primary care continues to grow, particularly as the ageing population increases. Join us next month on Wednesday, December 4, to explore challenges and opportunities to improve support for the health and social care needs of homebound older adults. Attendees also will hear about the current landscape of home-based primary care, including learnings from recent research and ways to build on CMS’s advanced primary care efforts.

Mortality and Antipsychotic Use in Dementia-Related Behavioral Disorders

Duke-Margolis and the FDA will convene a workshop to review data regarding risks associated with the use of medications in the antipsychotic class in older adults with behavioral and psychiatric symptoms associated with dementia. Workshop attendees will hear reflections on available evidence and further considerations related to the assessment of risks associated with the use of antipsychotics within this patient population.

Optimizing the Use of Real-World Evidence in Regulatory Decision-Making for Drugs and Biological Products – Looking Forward

Real-world evidence (RWE) holds the potential to advance regulatory decision-making in drug development. Duke-Margolis and the FDA will explore recent accomplishments and future activities for the FDA's RWE Program for drugs and biological products. Attendees will learn more about the FDA’s approach to RWE for regulatory decision-making, ongoing opportunities and challenges regarding RWE, and future initiatives for promoting the appropriate use of RWE.

Sixteenth Annual Sentinel Initiative Public Workshop



The Sentinel Initiative was launched in response to the FDA Act of 2007 (FDAAA) and the Congressional mandate that FDA develop a national electronic system to track the safety of regulated medical products. Since its inception, the Sentinel Initiative has continued to refine and expand its data infrastructure capabilities. During this hybrid workshop hosted under a cooperative agreement with the FDA, attendees learned more about the Sentinel Initiative's recent achievements as well as upcoming opportunities for the Sentinel System and BEST System.

From Principles to Practice: Exploring AI Governance in Health Systems


Duke-Margolis and Duke Health hosted a webinar to discuss details of a recently-released white paper, which explored how health care organizations can use governance to safely and successfully implement artificial intelligence (AI). Speakers discussed methods and supports to facilitate the democratization of AI governance so more health organizations can safely and responsibly use these novel tools.

Core Faculty member Kate Bundorf was selected to serve as a Provost Faculty Fellow for 2024-2025. This new program aims to introduce Duke faculty members to key leadership roles and responsibilities, and areas of strategic focus of the university. Congratulations, Kate! Read the full story here.


Kate and former Margolis intern Velda Wang (’25) co-authored a research article in Health Affairs that examined Medicaid coverage reporting in the wake of the COVID-19 public health emergency.

Will Bleser presented an overview of value-based payment and delivery reforms to address health-related social needs at the National Conference of State Legislatures’ event, “Addressing Social Needs in Value-Based Care Models.” His presentation also featured Duke-Margolis’s work to study North Carolina’s Healthy Opportunities Pilots program.

Mark McClellan was a speaker at the Health Care Payment Learning and Action Network (HCPLAN)’s 2024 Summit and discussed the upcoming opportunities and challenges for the transition to value-based care during a fireside chat.



The Milken Institute hosted its 2024 Future of Health Summit, where Mark moderated a session on clinical trial innovation. Learn more and watch the recordings here.


Mark also spoke at The Century Foundation’s event to explore how the election would impact health care in the United States. Watch the recording here.

The Veterans Administration awarded Core Faculty member Nathan Boucher a grant to pursue a randomized control trial of his previously piloted community health worker model serving family care partners of Veterans with serious illness across rural North Carolina. Nathan will be the Principal Investigator and Core Faculty member Brystana Kaufman will co-investigate with other experts.

Kun Li co-authored two papers:

  • JAMA Health Forum—the authors studied home health agencies and how change of ownership affected quality of care.
  • Health Services Research—the authors examined competition between community health centers and how it affected quality of services.

Frank McStay was a guest speaker on the Health Affairs This Week podcast and discussed the performance data for the Medicare Shared Savings Program (MSSP) as well as key findings from Duke-Margolis’s upcoming research on the annual performance of ACOs participating in MSSP.
