As part of the Ohio Pharmacists Association’s (OPA) Rural Health Transformation Pharmacy (RHTP) program, community-facing pharmacies are being invited to complete a needs assessment. Your pharmacy should have received or will soon receive an email with a site-specific survey link. Please complete the assessment by Sunday, November 8, 2026, using your pharmacy’s assigned link.
Survey responses will help OPA select pilot sites for the first year of the RHTP program and identify technical assistance and practical support needed by pharmacies in community settings.
If you believe your pharmacy has not received a survey link, please contact Myriam Shaw Ojeda (mshawojeda@ohiopharmacists.org) with your pharmacy’s name, the name of the pharmacist in charge, and a current email address where the link can be sent.
