Hello Everyone,

I'm happy to say that this newsletter is not for the purpose of sharing any outbreak news with you.

We continue to be outbreak-free.

As shared in the regular Family Newsletter sent out on January 3 2025, we are moving ahead with Project AMPLIFI. This is the program that increases the efficient and accurate exchange of information between the LTC Home and the Hospital whether we are sending or receiving a resident. This is a tremendous step forward in supporting smooth transitions in care and services for those navigating our healthcare systems.

First, we require your consent to move forward.

Residents who are their own decision-makers will be provided the opportunity to provide their own consent to this process.

POA's / SDM's for care, for residents, are asked to click on this link below, read, and if you have any questions, reach out to the Charge Nurse or Team Leader.

If you do not have any questions, please complete and sign the form. Once the form is submitted it comes directly to the nursing team at JMPC.

If you are struggling at all with the form, please speak with the charge nurse.

We are really looking forward to the benefits of this program and appreciate your support.

This line is available to raise concerns or ask questions related to LTC homes across Ontario and is staffed seven days a week from 8:30am – 7:00pm

The person who answers the call will take your name and contact information, ask a few questions on the issue, and forward this to an inspector for follow-up.

The complainant will hear back from the Ministry within two business days.
